Position Description : Assess individuals for long-term care options and for eligibility and enrollment.
Essential Job Functions:
  • Complete in home person centered assessments in order to provide options to clients and their care givers on potential programs and resources which can assist the client to meet his/her needs.
  • Complete appropriate referrals on behalf of the consumer.
  • Complete assessments for individuals applying for Waiver Programs, includes but is not limited to: Assisted Living, PASSPORT, Health Checks, Nursing Facility Placement, Etc. Depending on the program, assessments can occur in the consumers private homes, nursing facilities, hospitals, group homes, etc.
  • Complete Adverse Level of Care Assessments as required.
  • Develop an initial plan of care with consumers and caregivers, addressing the consumer’s presenting requests and needs.
  • Educate and assist individuals and his/her legal guardians, authorized representatives, and/or caregivers, on the following:
    • Program eligibility criteria
    • Basic information about Medicare, Medicaid and LTC insurance, as applicable.
    • Program participation expectations of the individual and/or legal guardian
    • Individual rights and responsibilities as a participant in the program
    • Limitations and expectations of service providers
    • Appeal rights and process
    • Educate individual on resources available to meet needs not available through the waiver program.
  • Assist with discharge planning to link consumers and their families to resources and services.
  • Identify health and safety risks, assist in providing direction/resources to prevent harm to the individual. Advocate on client’s behalf.
Qualification Requirements:
  • Ohio licensed Registered Nurse or licensed Social Worker in good standing with the credentialing board.
  • At least one-year prior experience in medical social work, home health care and/or geriatrics.
  • A good working knowledge and understanding of issues related to the elderly population.
  • Knowledge of the individual, family, social systems and the individuals functioning in the aging process.
  • Ability to collaborate and problem solve in a self-directed team environment.
  • Knowledge of community resources within Southwestern Ohio.
  • Good customer service skills.
  • Ability to adapt to frequent change.
  • Good oral and written communication skills.
  • Good organizational and teamwork skills.
  • A valid driver’s license and reliable transportation for travel within the COA 5-county area.
  • Ability to travel within the state of Ohio for training.
  • Respect for the cultural, spiritual, racial and ethnic beliefs of others.
  • Excellent computer skills including Windows operating systems, MS Word, MS Excel, data entry and the ability to learn new software applications as needed.
