CalAIM Program Manager

Job Summary:

The California Advancing and Innovating Medi-Cal (CalAIM) is a long-term commitment by California's Department of Health System to strengthen Medi-Cal, offering Californians a more equitable, coordinated, and person-centered approach to maximizing their health and life trajectory. The goal is to extend support and service beyond hospitals and health care settings. CalAIM will offer Medi-Cal enrollees coordinated and equitable access to services that address their physical, behavioral, developmental, dental, and long-term care needs, throughout their lives, from birth to a dignified end of life.

The CalAIM Program Manager will oversee all programs operating under our CalAIM contract, including but not limited to:

· Enhanced Care Management

· Housing Transition/Navigation Services

· Housing Tenancy and Sustaining Services

· Housing Deposits

· Sobering Centers

· Recuperative Care

Reports to:

· Mental Health Program Director

Overview of Role and Responsibilities

  • Lead planning and implementation of CalAIM program initiatives, including Enhanced Care Management, Community Supports, including but not limited to the following types of services Housing Transition/Navigation Services, Housing Tenancy and Sustaining Services, Housing Deposits, Sobering Centers, and Recuperative Care
  • Monitors and evaluate effectiveness and efficiency of CalAIM service delivery system, organizational structure, staffing levels, and other internal operations including identifying and recommending alternative approaches or improvements; implementation of revisions, adjustments and changes
  • Maintains an awareness of state-level changes to the CalAIM program that require local implementation changes
  • Organize and lead internal departmental meetings and trainings.
  • Support program reporting needs.
  • Contribute to grant reporting and invoicing.
  • Supervise, train, assign and evaluate the activities of CalAIM staff.
  • Support departmental hiring, staffing, and budgets.
  • Internally liaison with other departments, including finance, operations, data, and other program teams.
  • Performs other duties as assigned or required

Expectations:

  • Knowledge and applied practice of HIPAA parameters
  • Professional and discreet interpersonal engagement with all established and prospective clients as well as fellow care team members
  • Sufficient knowledge and utilization of peripheral services which may be necessary to fully address client needs, as well as the community support services being offered in the particular county of operation
  • Continued sensitivity to and awareness of cultural and linguistic diversity
  • Honesty, transparency, and fluidity in all client engagements and engagement attempts
  • Awareness of social determinants of health
  • Astute knowledge of, and diligent effort made on assembling and following up on comprehensive assessments and care management plans
  • Understand and abide by all departmental and companywide policies and procedures
  • Comply with all safety and injury prevention policies and regulations
  • Able to work Monday- Friday 8:00am to 5:00pm, with occasional evenings and weekends required.

Preferred Experience/Minimum Qualifications

  • R.N. L.C.S.W., M.P.H., M.S.W. or closely related degree; and
  • A minimum of 3 years of non-profit or related experience is strongly preferred;
  • Experience with and knowledge of CalAIM populations of focus is essential to be considered as a candidate for this position. Current and anticipated populations of focus include: people experiencing or at risk of homelessness, people at risk of frequent utilization of emergency services or hospital care, those with severe mental illness (SMI), and people involved with the criminal justice system. ·
  • Knowledge of mental health, drug and alcohol treatment, homelessness and housing resources, programs, and the Continuum of Care in Santa Barbara County, is preferred;
  • Basic computer skills, including the ability to send and receive emails and summarize data in spreadsheets.
  • Proficiency in Google Suite (Mail, Docs, Sheets, Calendar).
  • Possesses a high level of tolerance and understanding for individuals who present for services with urgent multiple case management and health needs
  • Have or be able and willing to obtain CPR/First Aid training.
  • Provide proof of full COVID-19 vaccination
  • Have reliable transportation and:
  • A valid driver's license
  • The Ability to qualify for Good Samaritan Shelter insurance coverage

Successfully complete the following as a condition of hire:

  • Tuberculosis Test
  • Background Screening
  • Drug Test

Work Environment
The employee may be in contact with individuals and families in crisis who may be ill, using alcohol and drugs, and who may not be attentive to basic personal hygiene, health and safety practices. The employee may experience a number of unpleasant sensory demands associated with the client's use of alcohol and drugs, and lack of personal hygiene. The employee must be ready to respond quickly and effectively to many types of situations, including crisis situations and potentially hostile situations. The noise level in the work environment is usually moderate in an office setting. Sometimes work may become stressful when working under pressure.
