GENERAL SUMMARY
Care Navigator (CCBHC) will provide a range of direct services to assist and support individuals in developing their skills to gain access to needed medical, behavioral health (including substance abuse), housing, employment, social, educational and other services essential to meeting basic human services; linkages and training for patient served in the use of basic community resources; and monitoring of overall service delivery, and complete associated documentation for the individual served and program.
ESSENTIAL DUTIES & RESPONSIBILITIES

  • Provides trauma-informed care management services to assist individuals in sustaining recovery, and gaining access to needed medical, social, legal, educational, and other services and supports.
  • Provides rehabilitation services such as medication education; self-management; training in personal care skills; individual and family/caregiver psycho-education; community integration services; recovery support services including Illness Management & Recovery; financial management; and dietary and wellness education. Services may also include the provision of supported services such as housing, employment, and education, the latter in collaboration with local school systems.
  • Ensures Care management activities are carried out in keeping with the individual’s preferences and needs for care and, to the extent possible and in accordance with the individual’s expressed preferences, with the individual’s family/caregiver and other supports identified by the individual served.
  • Establishes and maintains relationship with individuals and their families or caregivers and build trust between individuals and service providers in order to effectively provide clinical services, assessments and develop person-centered recovery plans.
  • Assists the integrated care team to resolve problem situations and monitor case management by enhancing communication amongst all individuals involved in individual’s treatment.
  • Communicates with individuals regularly. Identify any barriers patients experience in accessing health care. Arrange for supportive services as needed. Coordinate follow-up care, transportation, coordination, etc.
  • Ensures clinical documentation is submitted in accordance with Center standards, and meets all designated requirements.
  • Educates patients about health and wellness issues relevant to their care, community health resources, insurance information, financial assistance, pharmaceutical services, etc. and link and refer individuals to appropriate services (internal and/or external) as indicated.

MINIMUM ENTRANCE QUALIFICATIONS
Education and Experience
  • Bachelor’s degree from an accredited university or college in a Behavioral Health field such as Social Work, Psychology, Counseling or a related field; or Bachelor’s degree with at least 27 hours of Behavioral Health course work

  • Complete the Qualified Mental Health Professional – Community Services (QMHP-CS) training within six (6) weeks of hire or transfer

Other Requirements
  • Must maintain a valid driver’s license and automobile insurance coverage, be able to travel as needed, and be able to meet on a consistent basis the driving record requirements of the Company’s auto insurance carrier if you drive your vehicle during company business
  • Must maintain required credentials and mandatory training requirements to ensure compliance with all State regulations and CHCS policies
  • Must be able to meet the physical requirements to complete SAMA and CPR training including lifting up to 12 lbs. and supporting up to 55 lbs.; bending, stooping and getting on and off the floor without assistance.
COVID-19 Mandate Requirements
Unvaccinated Staff options:
a. Get Vaccinated per the CMS Mandate.
b. Have a medical/religious exemption. Unvaccinated staff will need to test once a week at the employees own time/expense and must wear enhanced PPE to include an N95 face mask and a face shield.
c. Consumer facing staff DO NOT have the option to be unvaccinated without an exemption.
d. If the position is not consumer facing, staff members may continue to be unvaccinated, without an exemption and test once a week at the employees own time/expense and must wear enhanced PPE to include an N95 face mask and a face shield.
Vaccinated Staff options:
a. For employees who are vaccinated and consumer facing, we are continuing to require wearing an N95 mask.
b. For all other vaccinated employees, we are requiring a cloth, surgical or N95 mask be worn in all shared spaces.
c. Vaccinated employees may only go unmasked if in a single occupancy office
PREFERRED QUALIFICATIONS
  • One (1) year experience providing similar services
  • Bilingual (English / Spanish)
  • Experience with or knowledge of wellness-oriented trauma-informed care
SUPERVISION

  • Job has no responsibility for the direction or supervision of others.

PHYSICAL DEMANDS
  • Must be able to meet the physical requirements to complete SAMA and CPR training including lifting up to 12 lbs. and supporting up to 55 lbs.; bending, stooping and getting on and off the floor without assistance.
  • Must have adequate mobility that requires frequent walking, standing, bending, stooping, kneeling, reaching (vertical and horizontal), using fingers, hands, feet, legs and torso in various care.
  • The employee must be able to occasionally transfer a consumer.
