Career Planner - Senior Community Services Employment Program (SCSEP)
Job description
I. Provides Case Management Services
A. Recruitment and pre-screening of potential applicants
· Maintains regular contact with appropriate area referral sources.
· Interviews applicants, including self-referrals to determine eligibility.
· Collects documentation to verify eligibility.
· Assists in review of completed eligibility determination forms.
· Maintains pool of eligible applicants necessary to meet targeted program criteria by reaching out to county referral resources as necessary. IE: Senior Centers, Food Pantries, Housing authorities, ADRC’s,
B. Provide orientation on available services, career information, and job-seeking skills
C. Provide/coordinate career assessment to assist participants in determining interests, aptitudes, abilities, limitations and training needs, and/or refers to additional assessment as needed.
D. Assists individuals in developing employment/training plans and service strategies to address assessed needs
· Identifies appropriate training activities and/or job placement strategies based on assessment.
· Identifies needed supportive services and coordinates the provision of such services with appropriate providers.
· Provides ongoing advocacy to individuals enrolled in services.
· Keeps accurate and timely case notes in SCSEP case note database
· Enter enrollment data into DOL database
E. Assists participants in the implementation of individualized training/employment plans.
F. Provides participant follow-up to assess progress and assist in retention
G. Develop and/or maintain current relationship with Host Agencies of Job seekers through positive and productive communication
QUALIFICATIONS: Skills in office management, team leadership, public speaking; ability to assess vocational skills and needs of defined service populations; knowledge of community resources; knowledge of business needs and the economy in the geographic area assigned; effective interpersonal and counseling skills; group facilitation and organizational skills; ability to keep accurate records; ability to travel.
Work from home is an option. Serves the following counties: Barron, Chippewa, Dunn, Eau Claire, and Taylor. Travel required. Much of the case management can be done via online after initial enrollment.
Potential advancement into lead position.
This position offers only Single Health Insurance.
