JOB DESCRIPTION:
The Case Aide under the direction of the Program Director will be responsible for promoting effective community reintegration for individuals who have traditionally been unable to manage in less supervised settings. As part of our multi-disciplinary team the case aide will work closely with clinical staff to assist individuals with on-site rehabilitation and reconnection to community supports. Responsibilities include direct care; assessment; escorting to appointments; co-leadership of skill development and socialization groups; medication supervision; and, participating in clinical team meetings.
RESPONSIBILIITES:
Client/Service Delivery
· Participate in program related activities as directed by the Program Director.
· Evaluate skills and needs of each individual and make recommendation to Case Managers and Clinical Social Worker.
· Help implement individual service plans for each individual in concert with the Case Managers.
· Provide direct service in areas such as medication, money management, daily living skills, and behavior management.
· Maintain records as required by the Case Managers, Clinical Social Worker and Program Director.
· Submit records as required by the Case Managers, Clinical Social Worker and Program Director.
· Participate in staff meeting as directed by the Case Managers, Clinical Social Worker and Program Director.
· Assist in setting up appointment and providing transportation for individuals.
· Participate as a team member in planning, implementation, and evaluation of services provided.
· Follow all policies and procedures relative to this position.
Team/Staff Relations
· Work as a Providence House team member with all staff and volunteers in order to implement the mission and core values of the agency.
· Attend regular internal and external agency meetings.
· Work as part of team to plan and develop residents programs.
Administrative
· Keep accurate, up-to-date documentation on all clients. Including but not limited to, treatment plans, case notes, etc.
· Complete all paperwork in a timely and accurate manner that ensures payment to the agency from various sources.
· Assists with any and all data collection systems.
Sanctuary Model
· Effectively use and practice Sanctuary Models tools; safety plans, community meetings, psycho-educational groups, team meetings, red flags reviews, and self-care in the program to create a non-violent and trauma sensitive culture.
· Promote and model the core values of Providence House Mission (Compassion, Non-Violence, Hospitality, and Community) and the Seven Sanctuary Model Commitments to create a non-violent and trauma sensitive culture.
· Integrate the Sanctuary Model SELF (Safety, Emotional Management, Loss and Future) framework into daily practice to ensure quality care and an effective work environment.
REQUIREMENTS
· Associate’s Degree in Mental Health/Human Services Preferred.
· Minimum requirement: High School Diploma/GED, with at least two years of experience working with the formerly homeless, mentally ill population.
· Experience with long-term homeless, low income and diverse populations.
· Excellent verbal and written communication skills and the ability to work independently.
· Competent technology management skills, including proficiency with using Microsoft Office; and knowledge of client database management.
WORK SCHEDULE
· Full-Time. Tuesday - Saturday, 9am - 5pm or a mutually agreed upon work schedule.
. Occasional evening work may be required.

SALARY AND BENEFITS
Salary $37,700.00 Annually. Benefits include 12 paid holidays, 2 weeks of vacation, 3 personal days, 12 sick days, birthday day off, early holiday closing hours, medical, dental, vision, prescription coverage, life insurance, short-term and long-term disability benefits, 403(B), pension benefits, monthly medical stipend stipend and an Employee Assistance Program (EAP).
