About Your Role:
- As a Case Manager, you will be providing in-home services for at risk families and youth
- As a Case Manager, you will complete risk and safety assessments, progress notes, service plans, quarterly reports, discharge reports, provide court testimony in regards to observed and documented family progress, barriers to service delivery and goal attainment
- Participate in staff development and training sessions as well as regular supervision and other job duties as assigned.
- Provide hands-on supervised visitation that promotes family reunification. Provides parental education to expedite the reunification of families; development of skills to utilize during familial interactions (positive behavior reinforcement, de-escalation skills and creating and maintaining a routine for positive outcomes)
- Case Managers will role-model and educate parents and/or guardians to promote healthy, safe and happy home environments.
- This position comes with a $2,000 hiring bonus!
Do the Following Apply to You?
- Valid PA Driver’s License
- Bachelor’s Degree
- Associates Degree in a related human services field with at least two years paid experience working with children or families
