FULLY REMOTE with some Face to Face travel *****Must live in Cincinnati, OH
Eastern Time Zone and the hours for the position are 8am – 5pm M-F
Responsible for health care management and coordination of healthcare members to achieve optimal clinical, financial, and quality of life outcomes. Works with members to create and implement an integrated collaborative plan of care. Coordinates and monitors health member's progress and services to ensure consistent cost-effective care that complies with healthcare policy and all state and federal regulations and guidelines.

- Must reside in the Cincinnati area
- 0-2 years of Nursing with Case Management experience
- Must be a Registered Nurse for the state of OH
- A combination of experience and education will be considered
- Must have valid driver's license with good driving record and be able to drive locally
Job Description
The position is remote with some travel to Nursing facilities involved depending on COVID community infection rates. Travel is estimated approximately 2 days a week. Travel reimbursement provided.
