Abbott House is seeking an energetic case planner to join the Preventive Case Planner team. In this role, you will be responsible for delivering casework services to at-risk families.

The Preventive Case Planner is responsible for the delivery of case work services to at-risk families that have indicated reports of Child Abuse and/or neglect. Case Planners will help families develop the skills and resources needed to strengthen the family unit and prevent out of home and/or foster care placements. Case Planners will use a strengths-based, intensive case management approach to service delivery that includes providing direct supervision and counseling to the children and families, linking family members to appropriate community-based services, forming, and maintaining positive family relationships.

Key Responsibilities

· Provides casework and case management services to families.

· Conducts regular home visits both scheduled and unannounced, during these visits worker must meet with children and parents.

· Conducts a formal home environment assessment to ensure dwelling is safe and address any safety concerns noted.

· Conducts evidence-based assessments that are specific to the family,

· Assess and monitor safety, imminent risk, and well-being of children,

· Ability to engage families and work collaboratively to establish service goals/plans.

· Responsible for making referrals to court ordered programs, monitoring progress, attendance, participation, and completion.

· Ensures family is seeking medical treatment for the child’s well-being and to address specialized or chronic medical conditions. This includes having telephone and face to face contact with service providers,

· Co-facilitate parenting classes,

· Responsible for educational advocacy of children that need an appropriate school placement, attend all relevant school conferences, and maintain a presence at the school.

· Responsible for linking families to the following community resources; after school/ daycare programs, recreational, job placement/ career services and housing.

· Responsible for providing progress reports for all court hearings related to the case.

· Attend and actively participate in regular meetings with Dutchess County Children and Family Services.

· Responsible for weekly supervision with Supervisor. A case planner will be prepared to review cases assigned.

· Completion of all documentation in a timely manner; assessments, progress notes, FASP’s, court reports, case summaries and any other paperwork needed.

· Attend training opportunities to enhance clinical skills and professional development.

· Ability to work non-traditional hours, including evening and weekends to accommodate the families schedule, available for on-call emergencies and support.

· Any other related duties as required

Qualifications:

Master’s Degree in Social Work or in a related field

Bachelor’s in Social Work or in a related field and at least 3 years of experience

· Minimum of two years of case management experience

· Excellent counseling, communication, interpersonal and writing skills.

· Knowledge of community agencies within Dutchess County

· Excellent writing and communication skills. Must be computer literate.

· Valid Driver’s License

Schedule:

Mon-Fri 9 AM to 5PM traditionally but may include evening and weekends to accommodate the families schedule, available for on call emergencies and support.

Salary Range:$ 40,000.00 to $45,000.00 Annually
