Program Description
Serving adults with serious mental illnesses, the Adult FSP Program provides an intensive array of recovery-oriented services and supports, including housing and basic needs assistance, physical health care, benefits assistance, employment services, and integrated mental health and substance abuse treatment services. The TAY (Transitional Age Youth) FSP Program assists seriously mentally ill 16-25 year olds in becoming independent and productive members of the community. TAY FSP provides mental health and substance abuse treatment, linkage to physical health care, housing assistance, and vocational and educational support to San Francisco’s most seriously at risk young people.

Job Description

The Clinical Case Manager evaluates the needs of clients who are living with mental, emotional, and/or substance abuse problems and develops a treatment plan in coordination with providers, family members, and support systems. Treatment plan typically includes coping skills development, psychiatric medication management, and linkage to basic needs. Clinical Case Manager helps clients in applying for and obtaining other social services while providing a safe and supportive environment for agency’s clientele and adhering to laws regarding confidentiality and reporting requirements. Services are provided both in the field and in the office and include individual and group therapy, crisis intervention, case management, client advocacy, and education. The case manager must maintain accurate detailed observation and progress records for electronic billing data entry. Referrals for the TAY and Adult FSP caseload come from San Francisco Behavioral Health Services and the caseload is about 17 clients. During the Covid pandemic, outreach to clients in the field and face to face interactions are still taking place.

Program Specific Responsibilities
  • Provide case management, mental health services, and crisis intervention for chronically mentally ill adults and transitional age youth
  • Develop and maintain good clinical relationships with clients
  • Collaborate with community partners in housing, including Tenderloin Neighborhood
  • Development Corporation, Larkin Street Youth Services and Community Housing Partnership
  • Provide case management and independent living skills education to clients in the community and in the office
  • Rotate being on-call support to clients after hours/weekends (cell phone support only, not face-to-face)
  • Responsible for 25 hours of direct service to clients per week
  • Maintain timely documentation of services provided
  • Carries a caseload of 17 clients
  • Provide officer-of-the-day responsibilities as part of the team
Minimum Qualifications:
  • Master’s degree in Social Work, Psychology or related fields of study required
  • Experience in case management and mental health services with severely mentally ill, dually diagnosed adults and transitional age youth a plus
  • Experience with Medi-Cal billing/documentation a plus
  • Working knowledge of word processing (i.e. Microsoft Word, Excel and Access)
  • Current BBS registry. Individual and group supervision is offered for AMFT, ACSW, and/or Registered Psychologist
  • Knowledge of the MHSA philosophy of care a plus
  • Willingness to work from a strength-based, harm reduction, client centered model with an emphasis on evidence-based practices
  • Strong organizational, communication, time management, and interpersonal skills a must.
  • Ability to maintain accurate and timely documentation of clinical work
  • Ability to thrive in a fast-paced environment
    • Bi-lingual skills a plus
