What are we looking for?

On behalf of our home health agency client, one of the largest agencies in the Phoenix Area, we are looking for a Clinical Manager. The Clinical Manager will be responsible for the overall supervision and coordination of clinical services. Coordinates and supervises an interdisciplinary team of staff to assure the continuity of high quality care to home health patients assigned to the team's area in accordance with physician prescribed plan of care, and all applicable state and federal laws and regulations.

Responsibilities

  • Manages and directs a team of Field Staff ensuring safe, effective, and appropriate home care services.
  • Receives referrals, determines services required, and Agency's ability to meet needs. Oversees assignment of appropriate clinicians to cases.
  • Meets with Clinical Staff on a regular basis to provide guidance and information related to specific issues. Provides direction to teams to assure that client needs are met and services are provided according to the plan. Assists clinical staff in establishing priorities, setting goals, and evaluating progress toward goals
  • Responsible for ensuring the assessment of patient needs and ensuring the plan of care is developed, implemented, and updated through coordination with physicians, hospitals, skilled nursing facilities, case managers, and other referral sources to ensure accuracy in plan of care development
  • Available during all operating hours and provides oversight of all patient care services and personnel, including making patient and personnel assignments.
  • Reviews OASIS data collection to assure accuracy and consistency with requirements. Communicates with clinicians and provides training as indicated to achieve compliance with collection and reporting timelines.
  • Provides leadership to team and support staff in identifying Agency/client needs and opportunities for quality improvement.
  • May serves as back up in the absence of RNs.

Skills

  • A Registered Nurse or Nurse Practitioner
  • 2+ years of experience in health care supervisory role and/or 2+ years of home health care experience
  • Regulatory and/or management experience preferred.
  • At least two years of home health experience.
  • Licensed and/or state required, and federal requirements met to practice as a clinical professional
  • Demonstrates knowledge of the appropriate skills for communicating with all ages, especially the geriatric population.

Location

This role will be located at our client's office, close to the Phoenix Mountain Reserve.
