Summary:

The Clinical Pharmacist is responsible and accountable for the provision of safe, effective, and prompt medication therapy. The Clinical Pharmacist is responsible for delivering optimal medication therapy to patients with a broad range of disease states. In addition, the Clinical Pharmacist will make recommendations to the interdisciplinary team to assure that medications are being used safely and effectively. Pharmacist services include but are not limited to assessing patient needs, include disease specific characteristics into medication therapy, adjusting care according to patient response, and providing clinical interventions to detect, mitigate, and prevent medication adverse events.
The Clinical Pharmacist is excepted to provide medication and practice-related education/training and actively serve as preceptors for doctor of pharmacy students and residents.
This position requires the pharmacist to work rotating day and evening shifts Monday through Friday, holiday rotation according to departmental policy and weekends.
Experience/Education Requirements:
  • Graduate of an ACPE Accredited College of Pharmacy
  • Doctor of Pharmacy
License or Certification Requirements:
  • Must present evidence of, or eligibility for, Illinois licensure as a Registered Pharmacist by date of hire
  • Previous inpatient hospital experience preferred
  • Completion of an ASHP Accredited PGY1 residency preferred

Responsibilities:

Direct patient care:
  • Proficiently provides direct patient -centered care for patients
  • Indentifies high-risk patients and finds interventions to enhance patient care
  • Provides comprehensive and individualized care to assigned patients
  • Provides appropriate evidence-based, patient centered medication recommendations
  • Actively participates in interdisciplinary rounds when available
  • Act as a resource for the interdisciplinary te am by providing in-service and patient education when necessary
  • Participates in the management of medical emergencies
  • Performs discharge medication review, reconciliation, and counseling as appropriate
  • Completes patient chart reviews to identify, prevent, or mitigate medication related problems, improper medication or dose selection, sub-therapeutic dosage, over-dosage, adverse drug reactions, medication interactions, failure to receive medications, untreated indications, medications without an indication and treatment failures
  • Effectively communicates with other healthcare providers appropriate recommendations
  • Satisfactorily complete all required continuing education hours regarding the care of acute stroke patients.
Phamaceutical services throughout the medical center
  • Perform job duties in alignment with organizational goals
  • Be willing to learn and work in multiple different roles within the pharmacy
  • Evaluate and verify medication orders in the central pharmacy
  • Verify IV fills and Omnicell cart fills prepared by pharmacy technicians
  • Verify chemotherapy treatment plans based upon current NCCN guideline recommendations
  • Prepares and/or supervises the preparation of medications in the IV room both on site and off site via telepharmacy
  • Provides safe and timely medication therapy based upon patient age and needs
Pharmacy Operations
  • Ensures cost-effective medication use
  • Identify processes to prevent adverse events and optimize patient outcomes
  • Continuously stays up to date on new guidelines and changes in medication therapies
  • Precepts and teaches pharmacy residents and pharmacy students by maintaining requirements
  • Maintains proficiency in hospital computer systems
  • Maintains proficiency in decentralized pharmacy services and clinical pharmacy programs
  • Maintains proficiency in operational systems and central dispensing pharmacies
Pharmacy preceptors
  • Precepts and teaches pharmacy residents and pharmacy students
  • Meet the full requirements of an ASHP Residency Preceptor within three years of initial hire date
  • Maintains requirements for preceptorship detailed in ASHP PGY1 accreditations standards
  • Develops student, resident, and staff training experience/competencies
