GENERAL JOB SUMMARY

  • This position is responsible for managing and supervising end-of -life hospice patient services under the direction of the Clinical Services Director ensuring that nursing needs and compliance with administrative, policies, procedures and federal guidelines are met. The Clinical Services Manager will provide leadership and participation at IDT meetings for coordination of care as identified in the patient’s initial, comprehensive and updated assessments, continuous assessment, and the implementation of the interdisciplinary plan of care. The Clinical Services Manager assists the Clinical Services Director with strategic planning, budgetary processes, program development, personnel management, staff development, quality assurance, and fiscal planning. May have case management duties. This position may involve direct management of patient care.

ESSENTIAL FUNCTIONS

  • Ensure that Hospice philosophy of care as defined by state and federal regulations is appropriately implemented.
  • Acts as next-in-charge in absence of Clinical Services Director.
  • Provide supervisory functions such as participation in staff selection, orientation, coaching, evaluation, annual reviews etc. of those listed under “supervisory authority.”
  • Communicate established requirements to staff, update them with any new requirements, and hold them accountable for such requirements.
  • Provide for adequate orientation and professional development opportunities for staff.
  • Attend and ensure that assigned staff attends required education sessions as scheduled.
  • Provide guidance to staff in their daily communication practices.
  • Serve as a professional role model and mentor to staff by responding positively to change while supporting colleagues.
  • Collaborate and communicate effectively as part of a team to establish and maintain cooperative working relationships.
  • Execute an effective, supportive, sensitive approach in response the needs of staff, co-workers, patients and families.
  • Create schedules and organize work for staff ensuring proper distribution of assignments, efficient utilization of personnel, space, travel requirements, and facilities.
  • Monitor patient caseloads and ability to build and manage caseloads.
  • Monitor the level of resources utilized in the delivery of service and determine the appropriateness of resource consumption in those areas.
  • Oversee the maintenance of medical records, assuring accuracy, completeness, security, and time-terms for retention.
  • Facilitate in finding resolutions to customer service issues in a timely manner.
  • Assist the Director of Clinical Services with Strategic Planning activities.
  • Provide services necessary for the palliation and management of the terminal illness and related conditions, including interventions to manage pain and symptoms.
  • May have case management duties.
  • Carry out physician’s orders accurately.
  • Pronounce patient time of death.
  • Provide procedural techniques to evaluate human injuries and diseases including symptom identification and appropriate treatments.
  • Provide care in a respectful, dignified manner as identified in the agency’s patient’s right’s statement.
  • Observe and provide accurate, timely documentation of nursing measures, medication administration, and changes in patient’s condition.
  • Comply with infection control policies and procedures at all times.
  • Practice safety in the workplace.
  • Maintain patient confidentiality at all times.
  • Advocate for patient’s best interest.
  • Keep the Clinical Services Director fully informed.
  • Fluent in the English language.
  • Ability to position self to move about, ascend, descend or traverse, transport, operate, activate, use, prepare, inspect or detect, perceive, identify, recognize, observe, inspect and assess are required. Ability to convey, communicate, speak clearly and exchange information, converse with, discern, and express oneself.

JOB REQUIREMENTS AND QUALIFICATIONS

Education:

  • Graduation from an accredited RN program. BSN preferred.
  • CHPN preferred, or ability to obtain CHPN within first year of employment.

Training Requirements: (Credentials, licenses or certificates)

  • Current Arizona R.N. license.
  • Current CPR Certification.

Experience:

  • Three years of progressive nursing supervisory experience in home health or hospice preferred.

Knowledge Requirements:

  • Demonstrated knowledge in hospice philosophy, medical aspects of hospice care including recent research, trends and pain and symptom management.
  • Strong working knowledge of Medicare and ADHS regulations.
  • Knowledge and support of agency’s safety and OSHA policies.
  • Understanding of HIPAA Privacy Standards and related agency policies.
  • Demonstrated understanding and compliance with agency policies, including but not limited to documentation, continuing education, work schedules and required HR documents, State and Federal policies and procedures, laws and regulations.
  • Patient’s Rights.
  • Advance Directives.
  • Safety in the workplace.
  • Customer service principles.

Skills and Ability:

  • Structure and content of English language including the meaning and spelling of words, alphabetization, rules of composition and grammar.
  • Calculate general math computations.
  • Business and management principles, leadership techniques and appropriate coordination of people and resources.
  • Analyze information and evaluate results to choose the best solution.
  • Moderate computer skills in Microsoft Office applications.
  • Utilize proper body mechanics and safe working techniques.

Special Requirements:

  • Ability to operate a motorized vehicle and is auto insurable with minimum coverage as follows: $100,000 bodily injury per person, $300,000 bodily injury per accident and $50,000 property damage and have reliable transportation for work in the community and surrounding areas.
  • Ability to travel with-in the agency’s service area of 1.5 hours distance to/from the home office. Other travel may include occasional overnight trips to attend conferences etc.
  • On-call hours may be assigned to include nights, holidays and weekends. Hours may be flexible.
  • Participate in and complete all continuing education activities as assigned in a timely manner.
  • Follow all State of Arizona laws for Mandatory Reporting.
  • Adhere to Hospice of Havasu’s Standards of Behavior.
