QUALIFICATIONS:

Demonstrate the ability to read, write and speak english in relation to completing the job duties appropriately. Must have a current listing as NAII for Division of Facility Services. Successful completion of a 90-day probation period in the facility. Aptitude towards learning material is needed to perform NA’s duties and work with the elderly population. Completion of a NA II course or experience equivalent is preferred prior to employment. Must be currently certified in the State of North Carolina. Must be able to follow written and oral instructions. Must have a courteous and tactful manner and have good personal cleanliness habits. Good physical, mental and emotional health and be strong enough to perform assigned duties.

WORKING HOURS: As assigned on schedule. May include but is not limited to weekends and holidays.

WORKING CONDITIONS:

Works in Well-lighted, well ventilated nursing units and rooms. May Stand, stoop, bend and walk intermittently during the day. May be required to handle emotional disturbances, may be subject to falls, burns from equipment, scratches, kicks etc, from residents. May have to lift residents and must be on constant alert for resident and self-injury

RESPONSIBILITIES:

Provides certified nursing assistant services to assigned residents in accordance with care plans, facility policies and procedures and at the direction of supervisor(s).

DUTIES:

  • Demonstrates operation of oxygen equipment as directed by nurse.
  • Demonstrates the ability to create a sterile field and maintain sterile technique.
  • Demonstrate the ability to change a sterile dressing for wounds over 48 hours.
  • Provide appropriate treatments to wounds over 48 hours as directed by supervising nurse.
  • Demonstrates the ability to perform male and female urinary catheterization using sterile techniques.
  • Provide care to a client with an established ostomy by obtaining a specimen, applying and removing appliances and providing routine site care.
  • Clock in and out on the timecard at the designated hour.
  • Report to nursing station promptly. Be in an appropriate uniform, complete with nametag.
  • Receive nursing assignment along with special instructions from unit charge nurse.
  • Receive report of resident care from off going employees as necessary.
  • Plan work assignment to meet resident needs (i.e., activities, visitors, residents.).
  • Wash hands before and after giving care to each resident.
  • Residents are to be kept clean and dry with special attention and care always given to the skin.
  • Performs routine tasks of caring for the residents. May take and record TPR, BP, and weights according to the assignment and resident need. Bathes residents or assists in self- bathing. Washes and combs hair as needed. Cleans hands and feet. Assists in oral care, shaving, and dressing daytime and nighttime.
  • All residents are gotten out of bed and encouraged to participate in resident's activities when medically permissible. Residents should be dressed in street clothes when up.
  • Resident rooms are kept clean and straightened and linen is changed as per routine and assignment of unit.
  • Correct any condition that may cause an accident (i.e., spilled water on floor, equipment obstructing hallway).
  • Carry out reality orientation when caring for residents who are confused or disoriented.
  • Answer all lights promptly. Keep the call light button within reach of the resident.
  • Check on all residents at least every thirty minutes.
  • Turn residents at least every two hours and always maintain proper alignment to assist with promoting skin integrity.
  • Encourage residents to take fluids unless contra-indicated (i.e., NPO-fluid restrictions). Ice water and juice are offered every two hours unless contraindicated.
  • Give care in a professional manner, kindness, respect, and courteousness to all residents ( i.e., call by Mr., Mrs., Miss.). The resident's right to privacy is respected by all.
  • Serve correct meal trays promptly. Feed residents who cannot feed self. Give partial help to those who need it. Encourage residents to go to the dining room when able and assist in transporting as necessary. Wash hands and face before and after each meal.
  • Work cooperatively with fellow workers.
  • Report to nurse in charge any change in resident's condition or an unusual condition noted regarding resident. Complete special nursing assignments made by the charge nurse.
  • Attend planned staff meetings and in-service programs for nursing assistants.
  • Make final rounds on residents, leaving them clean and dry. Rinse soiled linen and place in linen hamper in soiled utility room. Place under pads in hamper to be sent to laundry.
  • Record on appropriate records: Intake and output, turning of residents, TPR and BP and any other care given. Document all care given appropriately and timely.
  • Assist residents to schedule activities and the dining room for meals.
  • Assist residents with ambulation, crutch walking, using a walker and maneuvering wheelchairs.
  • Assist residents for naps.
  • Assist residents to and from bathroom and offer bedpans.
  • May assist in admitting and discharging residents as delegated by Licensed Nurse.
  • Prepares residents for tests and x-rays, visits by physicians and dentists, physical therapy, and to other areas of the facility and transport them to areas or departments where the above takes place. May assist with physical therapy.
  • Performs personal services for residents within reason such as reading to them, writing letters for them, operating TV and radio, assisting them with recreational and occupational therapy projects in their rooms, and doing whatever possible that will promote comfort, security and rehabilitation.
  • May assist with and/or perform procedures as outlined, with proper instruction and supervision, such as: admitting, transferring, discharging residents, taking and recording vital signs, measuring and recording intake and output, gives baths, gives special care to residents in casts, cares for residents in isolation, may give exercise and massage, assists with care of drying and dead, sees that safety devices are in use and safety precautions are followed, sees that room temperatures and humidity are compatible with residents needs, assists nurses in giving treatments and examining residents.
  • Applies aseptic technique in all resident care to prevent spread of disease and infection.
  • Assists with restorative and rehabilitative care of resident.
  • Performs assigned clerical duties at nursing station, answers lights, may answer telephone and intercom, runs errands as directed by nurse in charge.
  • Participates in ward or unit cleaning duties which are not performed by housekeeping personnel, as directed.
  • Provides input to licensed nurse regarding Plan of Care.
  • Performs other duties as assigned by supervisor within the NAII scope of practice.
