Job Title: Community Health Worker (Promotores De Salud), Kings County Healthy Communities Project (Part Time)
SUMMARY
Under general direction of the Program Manager and Community Engagement Coordinator, the Community Health Worker or Promotor De Salud supports the Community Engagement Coordinator with engagement and coordination of community outreach activities including COVID-19 Polymerase Chain Reaction (PCR) testing and vaccine events in Kings County. The Kings County Healthy Communities Project aims to implement services and interventions supporting the prevention and spread of COVID-19, particularly among underserved populations most affected by COVID-19. This will be accomplished by coordinating and participating in community outreach activities and events, hosting COVID-19 PCR testing and vaccination events, and developing a Promotor/Community Health Worker Education Network. Additionally, this role will support in creating health and community linkages to the public based on social determinates of health (SDoH) screenings conducted by the program.
ESSENTIAL DUTIES
1. Consults with the Program Manager and Community Engagement Coordinator (CEC) relative to planning and program direction.
2. Responsible for creating safe and inclusive environments for Kings County residents and screening participants.
3. Assist the CEC with the development and implementation of a community outreach plan that outlines community outreach instances and community linkages for Kings County residents to connect them with resources and reduce health and SDoH disparities.
4. Conduct community outreach and health education around COVID-19 and act as a bridge to COVID-19 testing, vaccine, referrals and services that advance social determinants of health.
5. Supports with COVID-19 outreach and education in Kings County neighborhoods.
6. Maintain a log of activities and collect data necessary for program reporting, attending community testing and vaccination events.
7. Screen and refer community members to services, resources, and information based on their stated needs and maintain case management reports to ensure residents are connected to needed resources.
8. Attend monthly trainings and meetings with other program and subcontractor staff to determine successes, challenges and opportunities for growth.
9. Develop community partnerships to support the goals of the program, including establishing relationships with program subcontractors, community members, and organizational partners.
10. Represents the California Health Collaborative at meetings with committees, community agencies, care providers and related entities, and facilitates inter-agency linkages, communication, and collaboration.
11. Performs other duties as assigned.
EDUCATION AND/OR EXPERIENCE
Lived experience in or close connection to Kings County communities; experience or a deep interest in community education/outreach; desire to learn and participate in a Community Health Worker certification program; ability to work with people from diverse backgrounds and experiences; ability to work independently, including organizing and prioritizing activities
A valid California Driver’s License and proof of insurance.
LANGUAGE SKILLS
Ability to read, analyze and interpret technical procedures, procedure manuals, government regulations, technical articles, and general business periodicals. Ability to communicate clearly and concisely both orally and in writing.
REASONING ABILITY
Ability to work independently, exercise initiative and good judgment, solve practical problems and deal with a variety of variables in situations where only limited or no standardization exists, and interpret a variety of instructions furnished in written, oral, diagram or schedule form. Ability to create safe and inclusive spaces for all participants.
OTHER KNOWLEDGE/SKILL/ABILITIES
Ability to communicate effectively in both written and oral form.
Ability to travel and, if necessary, attend out of town meetings or conferences which could require overnight travel.
