Compensation (Pay): $16.25 to $23.56/hourly, based on experience.

Job Summary: Responsible for helping patients and their families to navigate and access community services, and other resources and adopt healthy behaviors. The CCC Liaison supports providers and Case Managers through an integrated approach to care management and community outreach. As a priority, activities will promote, maintain, and improve the health of patients and their family. CCC Liaison provides social support and informal counseling, advocates for individuals and community health needs, and provides services such as basic first aid, blood pressure screenings, and simple wound care.

Essential Duties and Responsibilities:
  • Responsible for establishing trusting relationships with patients and their families while providing general support and encouragement.
  • Assists families and patients in community and home settings as determined by established algorithm to remove barriers related to access services.
  • Works with families and patients from diverse backgrounds in reducing cultural and socio-economic barriers between patients and institutions. Builds family and patient capacity to address health and social issues through education, care coordination, and community outreach.
  • Provides advocacy and support as needed to remove barriers that inhibit families and patients from receiving enabling services as needed. This can include helping families and patients fill out paperwork for medical insurance, housing, WIC, etc.
  • Identifies trends in barriers experienced by families when seeking care and provides data to care team members and the population health department.
  • Knowledgeable about community resources appropriate to needs of patients and families.
  • Researches and classifies community resources that will be of value to families and maintains a community resource database.
  • Provides ongoing follow-up, basic motivational interviewing, and goal setting with patients/families.
  • Follows up with patients via phone calls, home visits, and visits to other settings where patients can be found.
  • Works closely with medical providers to help ensure that patients have comprehensive and coordinated care plans.
  • Attends appointments with PCP and specialists if needed by the patient or family.
  • Provides referrals for services to community agencies as appropriate.
  • Works with patients, care teams, and community partners to obtain, follow through on, and close social referrals.
  • Communicates with patients and care teams regarding the status of referrals and outreach efforts.
  • Helps patients connect with transportation resources and provide appointment reminders in special circumstances. Transporting patients based on Peak Vista policy.
  • Promotes and demonstrates the mission, vision, and Core Values of Peak Vista.
  • Develops and maintains good working relationships with all departments, the management team, medical staff, and leadership.

Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Education/Experience:
Education: High school graduate or equivalent required; Associate Degree in Business Administration or related field preferred. Successful completion of a Community Health Worker formal training program such as from a college or other educational institution is preferred. Medical terminology and/or background preferred.
Experience: Experience working in a multi-cultural setting. Experience working in a community-based setting for at least 1 to 2 years preferred.

Certificates and Licenses: EMT, Paramedic, or Certified Nursing Assistant preferred; Current CPR (BCLS) certification required. Valid Colorado driver's license and proof of insurance. Must meet and maintain driver criteria as prescribed by Peak Vista's insurance underwriter.

Computer Skills: Basic computer skills required; electronic health record (EHR) experience preferred.

Looking at computer screen continuously for long periods of time, hearing within normal range. Able to travel as required and meet patients at locations as needed.
