The Organization

North Los Angeles County Regional Center “NLACRC” is a private, non-profit agency that is contracted with the State of California, Department of Developmental Services, to provide services to individuals with intellectual and developmental disabilities. NLACRC is one of the largest regional centers in California and has proudly served the San Fernando Valley, Santa Clarita Valley, and Antelope Valley since 1974. We serve individuals across the lifespan from infants and toddlers, school age children, transition-age youth, to adults and aging adults.

SUPERVISION: Receives general supervision from Community Placement Program/Community Resource Development Program (CPP/CRDP) Supervisor.

The Position and Job Summary

May work directly with consumers with challenging service needs residing in restrictive settings, including Stabilization, Training, Assistance, Reintegration (“STAR”) facilities, Canyon Springs, Developmental Centers (“DC”), Institution for Mental Disease (IMD), Mental Health Rehabilitation Center (MHRC), out-of-state, or other restrictive residential settings. May involve working directly with the Consumer’s legal guardian, families, clinicians, service providers and/or other stakeholders to develop and implement a comprehensive service plan (individual program plan) addressing transition to the least restrictive and integrated community setting. Work includes service brokering and overall coordination, ensuring individually based, comprehensive and extensive transition planning to ensure successful community placement.

  • May provide case management duties, including initiating and responding to consumers, families, and stakeholders about appropriate diverse, integrated and least restrictive, community-based resources. Facilitate communication and collaboration with consumers, families, legal guardians, and outside agencies towards person-centered placement and community options. May work in collaboration with other specialist or service coordinator positions on the collaboration of alternative placement and community options. Identify appropriate resources while coordinating specific transition services to ensure successful placement.
  • Coordinate and conduct assessments and updates for assigned consumers with other departments, specialists, service coordinators or stakeholders. Coordinate aspects of community placement, transition medical and social security benefits with outside agencies identify and secure approval for all service requests, provide status updates to CPP/CRDP Supervisor.
  • Finds and arranges for services while also working directly with individuals, their families, clinicians, service providers and /or other stakeholders to develop and implement a comprehensive service plan (individual program plan) addressing transition and community relocation.
  • Coordinates both services and service providers that are necessary to implement a service plan aimed at successfully transitioning an individual to a lesser restrictive, person-centered setting. Coordinate referral and assessment activities with agencies that provide behavioral, medical, and social services, including income maintenance, Medi-Cal eligibility, housing, modification of the home environment, transportation and other services as appropriate for preliminary care-planning services in a community setting.
  • Facilitates completion of required documentation to secure housing and enrollment in other programs that are necessary to assist transitioning individuals to various community settings. Ensures discharge date is coordinated with various programs and services available in the community.
  • Identify resource needs for assigned consumers, locate or assist in the development of service providers through collaboration with the Community Services department. Participate in the Request For Proposal (“RFP”) process along with the CPP/CRDP team. Educate potential and current service providers on best practices in service delivery as it pertains to the least restrictive, person-centered environment.
  • Maintain consumer records and documents, including and not limited to, funding and billing records, Individual Program Plans (“IPP”), Purchase of Services (“POS”), Special Incident Reports (“SIR”), Client Diagnostic Evaluation Report (“CDER”), or other required reports, consistent with either statutory, regulatory, or business practices. Track transition activities and produce reports for presentation at CPP/CRDP meetings and other meetings as requested.
  • Develop and finalize comprehensive service plans based on the consumer’s preferences that produce outcomes consistent with the consumer’s IPP.
  • Serve as a resource to resolve transition issues.
  • Ensures that the proposed transition plan includes the principles of person-centered service planning and that funded services are culturally and linguistically appropriate.

Employment Standards

Any combination equivalent to or likely to provide the required knowledge and skills is qualifying. A typical way to obtain the knowledge and skills would be a Bachelor’s degree in a related subject and two years of related experience. A Master’s degree in a related subject may be substituted for experience.

Skills and Abilities

  • Knowledge of developmental or related disabilities, social service provision.
  • Assessment, program planning, customer service, strong organization and paperwork management, strong verbal and written communication, and ability to communicate complex material. Must work successfully with people from a variety of cultures. Must have knowledge of a variety of computer software applications in word processing, spreadsheets, database and presentation software (MS Word, Excel, PowerPoint, etc.)

Essential Requirements

A valid CDL and reliable and transportation, or acceptable substitute, required.

This position is non-exempt. Regular Salary: $25.14 - $27.07 per hour.
