We are currently hiring a Complex Care Program Coordinator based in Worcester MA. The Complex Care Program Coordinator will work with our care team to make a profound impact on the lives of underserved individuals and families living with complex and/ or chronic medical and behavioral conditions. The coordinator will support the smooth operation of the Complex Care Management Program (CCM).

Job Summary:

• Manage various letters, mailings, and other outreach communications to C3 patients.
• Compile and manage electronic files.
• Provide inbound call back-up or triage for the Care Team.
• Complete work within required time frames to achieve the goals established for the enrollee/family.
• Use ACO care management tools and technology.
• Ensure that all services are offered in a culturally and linguistically appropriate manner.
• Ensure that all needed accommodations are consistently made for members with disabilities.
• Perform other C3 and clinical program related duties as assigned by supervisor.

Job Requirements:

• Excellent Communication skills (written and oral)
• Proficient in Microsoft Office – Excel and Word
• Demonstrated knowledge of or experience working with the target community.
