Coordinated Specialty Care Clinician (CSW A/B)
Job description
The Massachusetts Department of Mental Health (DMH) is seeking a dynamic Clinical Social Worker to serve as the primary clinician for young adults enrolled in the Coordinated Specialty Care (CSC) program. The CSC program is an exciting new program primarily serving the Greater Fall River area to meet the needs of young adults and families experiencing First Episode Psychosis. The Clinical Social Worker will receive specialized training including Navigate to enhance skills to provide evidence-based individual and family therapy and care coordination services to assigned persons served
Please Note: Days Off: Friday and Saturday; work schedule 8:30am – 5:00pm will work one evening per week
Duties and Responsibilities (these duties are a general summary and not all inclusive):
- Works as primary clinician for assigned persons served and is accountable providing all duties of CSC team member as outlined in Navigate Team Members guide.
- Provides Individual Resiliency Training for assigned clients; utilizes other identified evidence-based/best-practice modalities to adjunct as needed.
- Offers Family support and Family Education Groups for assigned persons served.
- Provides care coordination to assigned caseload, including assisting with benefits, transportation and assisting with or providing referrals for any other life domain impacting overall health and wellness.
- Coordinates and completes psychosocial assessments by meeting with clients, family and other individuals needed for case documentation.
- Develops individualized treatment plans for each person assigned.
- Conducts ongoing assessments and actively participates in discharge planning; assumes primary responsibility for developing, drafting, implementing, evaluating, & revising overall treatment/recovery goals in conjunction CSC guidelines.
- Ensures documentation follows DMH, Joint Commission and agency standards.
- Offers community based clinical interventions to persons served and side-by-side consultation/direction to members of integrated team in order to ensure successful provision of treatment interventions.
Preferred Qualifications:
- Bilingual or multilingual
- LICSW
- Experience working with individuals with serious mental illness, behavioral or mood disturbances in an inpatient setting.
- Prior knowledge and training in Coordinated Specialty Care model and Individual Resiliency Training preferred
- Current & Valid driver's license necessary for this function

MINIMUM ENTRANCE REQUIREMENTS:
REQUIRED WORK EXPERIENCE: None. (See Required Education and Licenses.)
REQUIRED EDUCATION: A Master's or higher degree in social work.
LICENSES: Based on assignment, travel may be required. Those employees who elect to use a motor vehicle for travel must have a current and valid Massachusetts Motor Vehicle Operator's license or the equivalent from another state.
- Current and valid licensure as a Licensed Certified Social Worker or Licensed Independent Clinical Social Worker by the Massachusetts Board of Registration in Social Work.
