Summary:

Diet Technicians assist the Dietetic-Diabetes Care Team at CNY Family Care by performing support nutritional care and patient support tasks such as summarizing and maintaining records of patient’s diets, completing initial intake for patient appointments, and updating EMR as needed, organizing and assisting the Registered Dietitian (RD) in the development of diet and food preparation education handouts and forms, assisting patients to navigate access to care services such insurance, prior authorization, Patient Assistance programs, community resources, etc. Works cooperatively and communicates effectively within a multidisciplinary framework to actively promote holistic care for patients and families.

Duties and Responsibilities include the following:

  • Assists in screening patients at nutritional risk by rooming of patients for office visits interviewing patients to obtain diet history, limited nutrition assessment data, food preferences, and obtaining blood glucose and pump logs *
  • Collects accurate data such as history from patient, measuring and recording of accurate vital signs, review of medications, reviews of systems, chief complaint, and barriers to care *
  • Reviews medical record to obtain pertinent nutrition and clinical information. *
  • Evaluates data collected to signal attention to the RD and/or CDCES if patients are at nutritional or diabetes risks. *
  • Documents nutritional screening/assessment information into the system and appropriate assessment forms. *
  • Assists in appointment management of the Dietetic-Diabetes Care Team and assists in timely and accurate documentation, procedure codes as well as diagnostic codes to facilitate a smooth process for billing. *
  • Assists in directing patients for procedures/testing as ordered by the physician and/or the RD/CDCES. *
  • Gives patient and family information/education materials as directed by the RD/CDCES/Physician for the success of overall patient care. *
  • Assist patients in navigating access to care services such as insurance coverage, prior authorization for diabetes prescription and services, Patient Assistance programs, community resources. *
  • Assists the Director of Dietetic Services, along with other team members in program management and maintaining program certification *
  • Projects a positive image for the practice and acts as a role model for the mission of the practice. *
  • Maintains confidentiality at all times following HIPAA guidelines. *
  • Responsible for occasional evening or weekend work. *
  • Responsible for any other duties under the direction of the PCP or Registered Dietitian/ CDCES.
