Description of Position

Provide comprehensive pharmaceutical services to wide variety of largely medically under-served patients. This is a dispensing and INR clinic role with the expectation of being a residency and student preceptor. As part of the pharmacist-run chronic disease state clinics, the pharmacist provides specialized services to patients and providers to improve drug usage and therapeutic outcomes. Responsibilities also include coordinating with other NCHC pharmacy technicians and pharmacists and participation in provider and pharmacy student education.


How You Will Contribute

  • Deliver high quality pharmaceutical care in a clinic setting by dispensing medication and counseling patients in an accurate, timely, and courteous manner. Foster the pharmacist-patient relationship and motivate patient cooperation toward the delivery of care. Strive to improve patient safety in all aspects of performance.
  • Conduct contraindication and duplicate therapy checks in formulating recommendations to physicians to ensure quality and economical drug therapy. Review medication orders for their appropriateness with respect to patient data including age, height, weight, diagnosis, and other pertinent information.
  • Manage patients in pharmacist-run chronic disease state clinics including, but not limited to, anti-coagulation, immunization, HCV, and gestational diabetes.
  • Precept pharmacy residents and contribute to the training of all pharmacy personnel.
  • Assure compliance and completeness of reporting with adverse drug reaction and medication error programs. Record recommendations and interventions on the Pharmacist Good Catch form, CQI processes, and/or within the organizational compliance software.
  • Attend staff and inter-departmental meetings advised by Director of Pharmacy or Pharmacy Manager and participate in outreach and community activities such as health fairs.
  • Remain active and qualified as a ASHP PGY1 community-based pharmacy residency preceptor.

Minimum Qualifications

  • Graduate of an accredited School of Pharmacy.
  • Five years of experience in a clinical/dispensing setting or completion of an accredited clinical residency program.
  • Current Arizona License or eligibility for license required
  • Current BLS certificate

Preferred Qualifications

  • Board Certified Pharmacotherapy Specialist (BCPS) or Ambulatory Care Pharmacy (BCACP)
  • APHA disease state management certifications (MTM, Diabetes, Immunizations) Demonstrated ability to achieve positive patient outcomes in a similar environment
  • Previous experience as a preceptor or similar mentorship/educational role
  • Scholarly or other professional activity

Compensation and Benefits

  • Starting at $128,900 annually or more, depending on experience
  • Medical insurance starts Day One for less than $5 per paycheck!
  • 25 days of combined sick and vacation leave, plus 6 federal holidays, in first year
