JOB SUMMARY
Plans, schedules and provides early educational in-home services for an assigned caseload of infant/toddlers with special needs. Administers and documents all program assessments. Designs, implements and adapts activities for language, cognitive, fine and gross motor, sensory, self-help, and social/emotional development for each individual.
ESSENTIAL JOB FUNCTIONS
To perform this job successfully, an individual must be able to satisfactorily perform each essential function listed below utilizing their applicable expertise to address individualized child and family needs.

Leads or is an active participant in the interdisciplinary team members to develop the individualized plans and multidisciplinary services for assigned child
- Provide outreach services to children and families
- Administers and documents all program assessments and updates accurately to meet designed timelines
- Plans, schedules and provides early educational in-home services for assigned caseload of infant/toddlers with special needs according to family's desired outcomes.

Designs, implements, and adapts activities for language, cognitive, fine and gross motor, sensory, self-help, and social/emotional development, for each individual child utilizing best practices.

Models effective adult-child interactions and provides written information on parenting and child development, as needed. Provides parents with written weekly activities and effective coaching

Maintain child records completely and accurately to include Individualized Family Service Plans with full assessment of child functioning and goals, documentation of child progress in the program, records of required contact with collaterals and family members, documentation of arena assessments and documentation of required forms and regulations regarding intake and discharge of child

Complete billing documentation according to schedule established by the company
- Maintains confidential child records/files. Documents all phone and face-to-face contacts with the family and other team members.
- Monitors, documents, evaluates, and determines the child’s progress in collaboration with the family, assigned team, and immediate supervisory team.

Communicates and collaborates effectively with families and all professionals verbally and in writing. Completes and submits all require program and child paperwork accurately and in a timely manner.
- Adhere to professional standards and guidelines according to their respective disciplines
- Keep supervisor informed of progress of all children assigned
- Meet direct service requirements based on part time or full time status, as designated
- Conduct visits with child, family, and relevant collaterals in natural environments according to the needs of the child and family
- Lead or co-lead groups within Early Childhood standards according to program needs and design (both child and parent focused)
- Assist families with program transition at child’s 3rd birthday or as determined appropriate, including local education agency or other setting determined to best meet the child and family’s needs

Participate in team meetings, supervision and actively engage in professional development opportunities as well as in-service training as required
- Abide by all program policies and an applicable regulatory procedures of governing bodies
- Performs other related duties and activities as required
Education and Experience:
- Two years’ experience working with infants or young children and their parents preferred.
- Bachelor's degree preferred in Early Childhood Special Education and ND Teaching License.
Certificates, Licenses, and Registrations:
- Valid Driver’s License and acceptable driving record.
Other Skills and Abilities:
- CPR and First Aid certification within on month of hire.
