East County Housing Case Manager

Your New Role…
The Housing Case Manager provides housing-focused engagement and case management services with an interdisciplinary, multi-agency team to high-risk youth in East County. Responsibilities include screening, intake and assessment; housing application, placement and on-going coordination; crisis intervention; case planning and contracting; resource brokerage and referral; case collaboration and advocacy; follow-up and after care; provision of clinical and financial record keeping related to job duties. Provide backup floor coverage at the East County Youth Opportunity Center, episodically.
Knowledge & Skills:
- Ability and desire to work with diverse staff and excel in a multi-cultural environment
· Knowledge of housing barriers, navigation, and advocacy.
· Understanding of the issues of substance abuse, mental health, trauma, abuse and adolescent development including brain development.
· Must possess excellent communication, organization, creativity, flexibility, optimism, and exceptional interpersonal skills.
· Must have access to an automobile, a valid driver’s license, and active auto insurance on that vehicle
· Must be willing to be CPR trained and certified
Education & Experience:
· Bachelor’s Degree in social work, psychology, or related field.
· Minimum of two years of experience working with high-risk youth with case management experience preferred
- Prior experience engaging youth who face multiple barriers, preferably around contexts associated with exploitation
· Ability and commitment to working with sexual minority and culturally diverse staff, participants and community.
