Job description

We are seeking Registered Nurses to service our Assisted Living Facilities throughout the low country (i.e., Charleston, Berkeley, and Dorchester County only). This RN would service these Assisted Living Facilities in addition to our assigned Providers and Medical Assistants.

Daily Responsibilities include but are not limited to:

• Traveling to assigned ALF's
• Assisting assigned ALF's with any medical needs (i.e., orders, etc.)
• Assessing patients with any acute needs and facilitating a visit with the assigned Provider.
• Coordinating and Facilitating Telehealth Visits with Providers (including specialty visits)
• Medication Reconciliations
• Chart Audit Reviews/Updates
• Addressing Quality Measures
• Addressing Risk Factors
• Enrolling eligible patients into Chronic Care Management
• Capturing Chronic Care Management
• Strong Communication with assigned Care Team and ALF Staff.
• Promptly assisting your assigned Care Team with any medical needs (i.e., administrative duties, orders, vaccine administrations, etc.)

This is a salary-based position. It is a full-time 8-hr position (8:00AM-5:00PM) (Monday-Friday). Other incentives include a pay per visit, monthly bonus, and car package benefit. This position includes many other company benefits!
