Family Home Provider Coordinator

PURPOSE
The Family Home Provider (FHP) Coordinator is responsible for the direct oversight and growth of Cedar Lake’s FHP Program. This position will function as a member of the Cedar Lake Residences (CLR) team and may provide supplementary assistance to any individual served by CLR.
ESSENTIAL FUNCTIONS
  • Assesses the need for potential growth to build a solid FHP and Co-FHP provider network.
  • Completes interviews and home assessments for new FHP applicants and will streamline the matching process and increase operational efficiencies.
  • Conducts planned and unplanned visits monthly to ensure quality assurance and compliance standards are met by reviewing documentation for consistency and accuracy. Ensures all documentation is current and uploaded into OASIS and HRST software. Completes home evaluations to ensure health, safety, and welfare standards adequately promote a clean and comfortable environment for the individual supported
  • Conducts information and training sessions to certify new FHP homes under the guidance and regulations of the Supports for Community Living (SCL) Waiver and Cedar Lake’s policies and procedures
  • Collaborates with the Cedar Lake Foundation Team to increase awareness and education of the FHP service through information sessions and recruitment events.
  • Conducts quarterly network meetings and/or events for current providers for the purpose of building connections and increasing support
  • Drafts contracts and submits them to department director for approval
  • Submits stipends for each provider to the department director for approval
  • Attends and participates in the individual’s plan of care meetings
  • Other duties as assigned
QUALIFICATIONS
  • High school diploma or equivalent; some college education preferred
  • Must have a valid driver’s license and auto insurance
EXPERIENCE
  • Prefer 1 year of experience working with individuals with ASD/IDD
  • Strongly prefer knowledge and understanding of the SCL waiver and regulations
  • Some experience with public speaking and conducting training
WORK SCHEDULE
  • Full-time, 40 hours per week or more.
  • Typical work duties vary depending on the needs of the agency and may require longer workdays or weekends. Will include on-call responsibilities.

  • Hourly, non-exempt
PHYSICAL DEMANDS/WORKING CONDITIONS
While performing the essential duties of this job, the employee is required to:
  • frequently stand, walk, sit, reach outward
  • occasionally reach over the shoulder, squat or kneel, bend and climb.
  • (with or without assisting devices or with assistance from others) frequently lift/carry up to 20 pounds, occasionally to 50 pounds.
  • frequently push/pull up to 40 pounds
  • Other physical abilities required are near, close, distant, and peripheral vision (corrected) with depth perception and color recognition; a good sense of hearing, smell, and touch and the ability to wear personal protective equipment.
  • Working conditions may be at a day training program or business with a moderate noise level.
