General Summary of Duties:
Under the supervision of the Director of Family and Community Health Services, this position will provide supportive services to pregnant and parenting women and their families, through the Boston Healthy Start Initiative (BHSI). Responsibilities include but are not limited to intake assessment, service planning, service coordination and monitoring of patient progress from intake to discharge from services. The Family Partner will provide a full range of services for patients who are followed during their pregnancy, postpartum and interconception period by on-site hospital staff providers and will serve as a liaison with designated hospital.

RESPONSIBILITIES
1. 20 % Intake Assessment
- Provides universal screening to determine social service needs of the patients using a comprehensive risk-assessment tool.
- Develops and implement a process of reassessment for patients to determine on-going needs.
- Provides opportunity for patient input and education about potential needs.
2. 20 % Case Conferencing/Reporting
- Presents and participates in case conferencing discussions to facilitate the development of coordinated patient service plans.
- Organizes and maintains patient service records.
- Prepares internal and external reports to document patient care and services provided.
3. 20 % Services Coordination/Liaison
- Develops with the patient a coordinated service plan that includes goal setting, action towards goal and selection of resources.
- Establishes and maintains formal referral linkages with external as well as internal health care and community service providers that allow for delivery of comprehensive care, patient follow-up, and thorough resource options for patients.
4. 15 % Home Visiting
- Conducts home visits, as appropriate.
- Accompany patients to agencies and/or appointments, as necessary to provide guidance for access to health care or other supportive services.
- Counsel patients in the practice of effective negotiation to obtain services, documents and monitors services obtained and progress made toward services plan goals.
5. 10 % Community Outreach
- Participates in prenatal registration on-site at MCHC.
- Outreach to women in the community with purpose of getting women in to prenatal care as early as possible.
- Assists women in navigating the health care system for their children, encouraging primary care visits and immunizations.
- Develops and leads parenting groups for MCHC patients.
6. 15 % Provision of Direct Patient Care
- Performs patient care procedures such as, but not limited to, taking vital signs, weights, health screenings.
- Collects specimens from patients for laboratory test and notifies providers of results. May perform routine tests or urine samples and other specimens.
- Records in patients record all patient care provided and ensures that appropriate forms are properly completed for billing purposes.
- Maintains current level of knowledge and skills by attending and participating in continuing education programs.
- May serve on Health Center committees that promote Women’s health.
- Provide translation as needed.
Performs other related duties as required or assigned by the Family and Community Health Services Director.
Requirements:
This position prefers that the candidate is a Certified Community Health Worker or a graduate from an accredited Medical Assistant Program and possess a current certification in Cardio-Pulmonary Resuscitation (CPR) and Basic First Aid.. One to two years of clinical Medical Assistant experience in either a hospital or community health center setting is preferred. Excellent organizational skills, and Basic Computer operations skills such as Word, Excel as well as demonstrated oral and written communication skills are required. Knowledge and experience working with Electronic Medical Records (EMR) Systems such as EPIC OCHIN and experience in a community health setting with persons of diverse cultures is a plus. Bi-lingual in Haitian Creole and/or Spanish is preferred.

Pay: $55,000.00 per year
