Title: File Clerk Division: HOPICS

FLSA: Full-Time (Non-Exempt) Supervisor: Quality Assurance Manager

Pay Range or Rate: $20.00 - 24.00 Revised: 10.12.2022

Summary

Under the supervision of the Quality Assurance Manager, the File Clerk will file items in alphabetical or numerical order or according to the filing system used. This individual will grant access to the file room and file cabinets, as needed by specific staff. Locate and remove material from file when requested.

Essential Functions

  • Keep records of materials filed or removed, using logbooks or computers. Maintain order of the file room daily.
  • Add new material to file records or create new records as necessary.
  • Perform general office duties such as typing, operating office machines, and sorting mail.
  • Track materials removed from files to ensure that borrowed files are returned.
  • Gather materials to be filed from departments or employees.
  • Enter document identification codes into systems in order to determine locations of documents to be retrieved.
  • Sort or classify information according to guidelines, such as content, purpose, user criteria, or chronological, alphabetical, or numerical order.
  • Find and retrieve information from files in response to requests from authorized users.
  • Scan or read incoming materials to determine how and where they should be classified or filed
  • Place materials into storage receptacles, such as file cabinets, boxes, bins, or drawers, according to classification and identification information.
  • Assign and record or stamp identification numbers or codes to index materials for filing
  • Answer questions about records or files.
  • Modify or improve filing systems or implement new filing systems.
  • Perform periodic inspections of materials or files to ensure correct placement, legibility, or proper condition.
  • Eliminate outdated or unnecessary materials, destroying them or transferring them to inactive storage, according to file maintenance guidelines or legal requirements.
  • Operate mechanized files that rotate to bring needed records to a particular location.
  • Design forms related to filing systems.
  • Retrieve and disperse mail for clients.
  • Retrieve and fulfill request for documentation contained in clients file(s) and maintain log indicating date request was received and date request was fulfilled.
  • Assist with Department of Mental Health billing in the Integrated System
  • Confirm and print clients monthly Medi-Cal eligibility when asked.
  • Filing of billing information.
  • Categorizing billing information
  • Create English/Spanish Intake and Annual Packets, ensuring there is a minimum of ten packets available at all times (BHS file room specific)
  • Scan and upload documents in Clinitrak on an on-going basis
  • Create new client charts, including labels
  • Check client eligibility on a monthly basis in the IS and Medi-Cal Systems when asked
  • Regular attendance required. Other duties as needed and Overtime, holiday, or weekend work might be required.
  • Maintain appropriate boundaries; and adhere to SSG’s Code of Ethics and HOPICS’ Core Values.
  • Represent the Agency in a professional manner at meetings and community events.
  • Maintain files/records on client services in compliance with HIPAA & CFR42 and other funding requirements for auditing purposes.
  • Regular attendance required.
  • Other duties as needed.

Secondary Functions

Perform any other appropriate responsibilities as assigned by the Quality Assurance Manager. Other duties as needed and overtime, holiday, or weekend work may be required.


Non-Essential Qualifications - Knowledge, Skills and Abilities

Skilled at utilizing computer programs including but not limited to word, excel, access, etc.

Minimum Qualifications - Knowledge, Skills and Abilities Required

  • High school diploma or GED equivalent.
  • Verification of Employment Eligibility and Background Clearance.
  • Customer-focused attitude, with high level of professionalism and discretion.
  • Excellent communication skills, oral and written
  • Excellent organizational skills and attention to detail
  • TB test required (Not more than (3) months prior to or (7) days after Date of Hire, and renewed annually thereafter),
  • CPR and First Aid Certification required within 30 days of employment.

Mandatory Covid19 Vaccination Requirement

  • COVID-19 Vaccination Required: All staff is required to be fully COVID-19 vaccinated and to submit vaccination records to the HR Department during new hire orientation.
  • Fully COVID-19 vaccinated (2 weeks from last vaccination dose to be considered fully vaccinated)
  • Negative COVID-19 Test: All new staff must provide a negative COVID-19 test. PCR test results can be submitted prior to the first day of employment to recruitment@hopics.org. Rapid antigen tests are also acceptable; however, they will be performed onsite to verify results.

Supervisory Responsibilities:

This position does not have any supervisory responsibilities.

Work conducted in an office setting.

Physical Requirements

The File Clerk typically spends time sitting, standing, typing, thinking, writing, walking, carrying (max. 25 lbs.), listening, speaking, pushing, pulling, and working a flexible schedule. Must be able to move continuously during work hours.

Mental Requirements

This position will require the individual to be able to handle any/all the following: constant distractions, interruptions, uncontrollable changes in priorities/work schedules be able to process information, think and conceptualize.
