Summary/Objective

The Case Manager assists program participant access support services and provides referrals to services of the agency and community partners.

Essential Functions

  • Targeted recruitment of low income clients who receive TANF, are high school dropouts, veterans, and non-custodial and other single parents
  • Provide case management assessment and case planning to improve individual/couple, family functioning
  • Works in collaboration with the child support office
  • Provides case management for clients who enter employment for not less than one month
  • Monitors all domestic violence protocols
  • Facilitates the administration of client intake and evaluation
  • Administers psycho-social assessments
  • Provide referrals and receive referrals from community partners
  • Work with program coordinator in marketing activities
  • Maintains participant files in compliance with agency PII policies
  • Secures community venues for program activities
  • Directs work of volunteers
  • Assists in planning and developing client events

Supervisory Responsibility

Supervises any volunteers assigned to his/her classes or events

Work Environment

Work is normally performed in a typical interior office or classroom environment, limited exposure to physical risk. Extended periods of time sitting in front of computer.

Physical Demands

While performing the duties of this job, the employee is occasionally required to stand; walk; sit; use hands to type; reach with hands and arms; climb stairs; balance; stoop, kneel, crouch; talk or hear; The employee must occasionally lift or move up to 25 pounds. Employee will be moving audiovisual equipment and curriculum materials into venues and setting up rooms. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception and the ability to adjust focus.

Position Type/Expected Hours of Work

This is a full-time position. Days and hours of work are Monday through Friday, 8:00 a.m. to 4:30 p.m. must be willing to work some nights and weekends as required by program activities

Travel

Travel is primarily within the five county jurisdiction. However, out of area and overnight travel may be expected depending on program requirements. Must be willing and available to participate in required and/or professional development opportunities.

Required Education and Experience

Bachelor’s degree in social work or related field. Any combination equivalent to experience and education that would likely provide the required knowledge and abilities to be qualifying.

Preferred Education and Experience

Master’s Degree or LSW, case management experience, knowledge of local/state social service agencies. Understanding of generational poverty and low income populations.

Additional Eligibility Qualifications

Excellent computer skills (Internet, Word, Excel, PowerPoint, Use of Web Based Data Management Systems)

Additional Requirements

Valid Kentucky driver’s license, reliable transportation, and satisfactory background checks. Pass a pre-employment drug test and random drug testing as required. State required minimum vehicle liability insurance and uninsured motorist insurance. Any/all other testing or applicable checks required by local, state, or federal law.
