Elderly Services Program (ESP) Enrollment Specialist

Position Description: To provide support to care management teams and assist in the ordering of supplies, arranging services and ensuring necessary follow up for client services and referrals

Essential Job Functions:
  • Collaborate with the integrated care team to ensure all aspects of clients’ care are being addressed and monitored to promote quality, cost effective outcomes
  • Conduct home visits to assist clients with completing enrollment paperwork for services including, but not limited to consumer directed care, PASSPORT/waiver services, home modification
  • Relay concerns about client’s condition or home environment to Care Coordinator or other appropriate source.
  • Respond to requests from other members of the care team in a timely manner to maintain the health and safety of the consumer.
  • Arrange and/or award referrals for needed services which includes services covered both within and outside of the Long Term Care programs
  • Provide case updates as needed to clients, Care Coordinators, providers and other members of the clients care team.
  • Update service plan/care plan and notify Care Coordinator with service changes and additions.
  • Complete service suspensions
  • Direct clients to billing team as needed, follow process for delinquency of payment through suspensions and client communication
Qualification Requirements:
  • Minimum of one year of experience in social services or direct customer service or an equivalent combination of education and experience or be a Community Health Specialist.
  • Able to gain knowledge and understanding of issues related to the elderly population.
  • Knowledge of community resources within Southwestern Ohio.
  • Excellent customer service skills.
  • Good interpersonal communication skills.
  • Ability to adapt to frequent change.
  • Good oral and written communication skills.
  • Good organizational and teamwork skills.
  • Ability to collaborate and problem solve in a self-directed team environment.
  • Excellent computer skills, including the Windows operating and email system, MS Word, MS Excel, data entry and the ability to learn new software applications as needed.
  • Ability to travel within the state of Ohio for training.

Pay: starts at $16.00 per hour (based on education and experience)
