This is not a typical 8 am to 5 pm. We are supporting individuals 24 hours/day, 7 days per week so on-call is required and weekend work is not uncommon.

· Capable of effective communications with all sources of contact both within the facility (including employees and clients) and at the various agency, community levels as well as with parents and guardians.

· Manages activities of each individual’s Person Directed Plan (PDP) to ensure assignments and deadlines are met.

· Meets at minimum once a quarter with individual and supports.

· Coordinates programs and ensures compliance with the definition in the HCS Service Definitions and Billing Guidelines for individuals receiving HCS funding.

· Assists Administration in implementing a teaching and training philosophy emphasizing improved, independent functioning for each individual.

· Assist with monitoring performance management of staff (including evaluations, developmental plans, progressive discipline, etc.).

· Ensures the rights of the individual or the individual’s rights as exercised by the LAR on behalf of the individual are protected.

· Ensures that the individual, the individual’s LAR and family members, with the consent of the individual or the individual’s LAR participates in making choices about where the individual will live, attend school, work and take part in leisure activities.

· Coordinates the development and implementation of the individual’s PDP and justifications of service.

· Coordinates the delivery of the individual’s IPC.

· Advocate for the individuals supported and provide premiere services and solutions for those individuals which allows for equal opportunity, self-direction and independence

· Integrates various aspects of services delivered under the Residential and HCS Program and other sources.

· Performs an audit on random charts monthly.

· Assists with billing and navigating the appropriate billing systems and files all required documentation accordingly for each client.

· Ensure compliance with licensing standards year round. Assist program managers to prepare for licensing, attend inspections, answer requests of licensing personnel, and follow-up to licensing deficiencies. Act as a liaison with other agencies regarding licensing issues. Performs monthly site visits for assigned clients at program sites to address client needs.

· Performs quarterly home audits for all location including but not limited to individual homes, residential homes, and Host individual if applicable.

· Follows Texas Department of Family Protective Services and Adult Protective Services guidelines and regulations regarding Abuse, Neglect and Exploitation.

· Oversee residential staffing and performance

· Ensures HIPPA standards are followed at all times and client confidentiality is kept as well.

· Visit, interview, and report on potential consumers. Develop letters of intent and ancillary materials as directed

· Act as agency representative in the community at special events or meetings.

· Other duties as assigned by supervisor.
