Health Department Program Case Manager

Normal Work Hours/Days: 8:00 a.m. – 5:00 p.m. – Monday-Friday. Some flexibility dictated by patient need will be required.

The Health Department Program Case Manager II position is a direct service provider and an information and referral agent for members of the Target Population (seriously mentally ill) who reside in local area boarding homes and as assigned by supervisor. This position focuses primarily on referrals form Jefferson County Department of Health, as well as referrals received from the community at large, as assigned by supervisor. Clients served by this employee will often be acutely ill and/or in need of immediate and time-limited assistance.

PRIMARY JOB FUNTIONS:

Provide access to a wide range of services for the seriously mentally ill as assigned by the supervisor.

Properly assess need and devise individualized service plan for each client.

Develop and maintain a working relationship with Health Department employees and owner/operators of local boarding homes.

Serves as a liaison between the client and various service providers to ensure continuity of care.

Effectively implement written service plans to meet client goals and objectives.

Develop and maintain adequately detailed and accurate client records.

Provide transportation to clients when necessary.

Provide accurate documentation in debriefing for weekly and monthly reports/meetings as stipulated by program demands.

Perform all administrative duties as required.

MINIMUM QUALIFICATIONS:
Bachelor of Arts or Bachelor of Science in a Human Services field from an accredited university. Alabama State Board of Social Work Examiners’ License or licensure eligibility preferred. Minimum of one (1) year experience in social services/human services field required.

Valid Alabama driver’s license and a suitable automobile to permit the required travel. Must show car insurance equal to or in excess of the State of Alabama standards for automobile insurance coverage.

KNOWLEDGE, SKILLS AND ABILITIES:
Knowledge of seriously mentally ill Target Population definitions.

Ability to work autonomously.

Considerable knowledge of the special needs and behavioral characteristics of the seriously mentally dysfunctioning adult population (target population).

Willingness to work in area boarding homes in which the seriously mentally handicapped population may be a resident.

Extensive knowledge of the community and available community resources.

Knowledge of the rules and regulations governing the human services provider agencies in the catchment area.

Knowledge of the legal and ethical issues relative to confidentiality of client records and the ability to interpret and apply them within the scope of the case management responsibilities.

Demonstrable ability to communicate effectively with a multi-disciplinary team.

Considerable knowledge of casework principles and methods related to case management.

Working knowledge of interviewing techniques and principles.

Ability and willingness to document activities and maintain records.

Skills in interpersonal relationships and dynamics.

Considerable written and expressive communication skills.

Acquainted with the use and side effects of psychotropic medications.

Demonstrable ability to work independently and to make sound judgments regarding client care.
