Elmora Hills Healthcare and Rehabilitation is a 200 Bed Skilled Nursing Facility conveniently located in Union County is seeking an External Liaison. Our ideal candidate will create and maintain working relationships with case managers, doctors, and social workers of area hospitals, and facilities to help promote and foster referrals. You will also meet with prospective patients and families, and determine if patient meets sub-acute criteria, predetermine on-site evaluations as needed.

Other duties are as follows:

• Attend hospital discharge planning meeting as requested, representing the facility.
• Assist with giving tours to potential patients and/or families as well as to insurance representatives.
• Along with the Director of Nursing and Therapy Coordinator, evaluate the need for any specialized therapy or equipment, prior to admission.
• Review transfer forms from hospitals to evaluate appropriateness of admission to rehab unit and discuss same with the Director of Nursing and/or Therapy Coordinator.

Pay: $60,000.00 - $90,000.00 per year
