  • Position Overview:

High Fidelity Wraparound is a planning process that follows a series of steps to help children and their families realize their hopes and dreams. A High Fidelity Wraparound Facilitator is assigned no more than a caseload of 10 (to ensure availability to complete the process as designed). The High Fidelity Wraparound Facilitator will utilize the skills learned through National Wraparound Implementation Center training to provide wraparound services to children and families. The High Fidelity Wraparound Facilitator will combine understanding of children’s behavioral health and cultural sensitivity to partner collaboratively with families, their supports, other professionals, and community partners to develop individualized, family determined, strength-based plans of care.

This position is full-time with a regular, but flexible, schedule. The High Fidelity Wraparound Facilitator is responsible for the coordination of community-based services that are being provided to individuals that are assigned to their caseload. High Fidelity Wraparound Facilitators will assist families and youth in gaining access to, and receiving, needed social, medical, educational, vocational, residential, and other services. These are discovered with the family and by the Child and Family Team and then documented in the Plan of Care. This position may also be responsible for a variety of tasks that are nonetheless essential under the heading of “Miscellaneous Duties”. Immediate administrative and clinical supervision will vary by program assignment; however, ultimate local supervision will be through an assigned Site Administrator or Program Coordinator.   • Major Tasks / Responsibilities

  • Direct Services
    • Conducts assessments/reassessments according to program or funding stream requirements. Develops family stories and presents them to families and team partners for understanding.
    • Conducts monthly Child and Family Team meetings to develop program and consumer specific service plans.
    • Monitors the implementation of Plans of Care.
    • Arranges for and assists assigned consumers in accessing needed services.
    • Develops and maintains appropriate and supportive relationships with consumers, guardians, and, as appropriate, their significant others, formal, and informal supports.
    • Monitors progress, services, plan effectiveness, health, safety, and welfare of assigned consumers.
    • Advocates on behalf of assigned consumers.
    • Enhances consumers’ natural support systems.
    • Assists in consumer crises and crisis planning.
    • Conducts visits with the consumers and team partners in their home, school, work, etc. as deemed appropriate.
    • Makes presentations to committees according to program requirements.
    • Provides prompt follow-up regarding service commitments.

  • Programmatic Responsibilities
    • Ensures consumers continue to meet program eligibility requirements.
    • Acts as a liaison to referral sources.
    • Ensures other service providers have current plans and other paperwork as program requirements mandate.
    • Ensures compliance by other service providers to the consumers’ Plans of Care.

  • Administrative Responsibilities
  • Documents services provided according to program specific guidelines and corporate policies.
  • Prepares and completes necessary data reports.
  • Completes all employee related paperwork in an accurate and timely manner.

  • Miscellaneous
  • Meets productivity standards as assigned.
  • Works cooperatively with other program professionals to facilitate smooth inter-program functioning.
  • Maintains any resource or other informational files as assigned.
  • Attends all required meetings, coaching sessions, and trainings necessary for the HFW process. This includes completion of the 12-hour Core Components of the Targeted Case Management Curriculum, as well as the six-hour Specialized Curriculum for Children Diagnosed with Severe Emotional Disability (SED). This also includes completion of Introduction to Wraparound, Engagement, Intermediate, and any other required HFW specific trainings.
  • Performs other duties or responsibilities as needed or assigned.
