Presbyterian Homes & Services - Optage Home & Community Based Services program is seeking a Home Care RN Case Manager to join its team serving clients in our West Metro Region.

  • This role is a Full Time Salaried position
  • General hours are Mon-Fri, 8AM-4:30PM
  • This includes working one weeeknd day approx every four weeks and every fourth week on call which is attached to the weekend.
  • No Rotating Holidays but you are on-call.
  • Heavy focus on wounds, cath care, education on chronic conditions, medication set ups, case management.
  • Must be comfortable working independently
  • West Region:
    • Would be serving in the community and in PHS communities. The main PHS communities would be: around Lake Minnetonka Shores, Shorewood, Beacon, Chan, Folkestone and surrounding areas.

PHS Home Care began in 1980 and currently serves 5,000+ clients per year.
  • In this role, you will get to build relationships with your clients and will play a vital role in optimizing their independence!
  • In this role, you will receive calendar flexibility by being able to schedule client visit times.
  • In this role, you will get to provide skilled nursing tasks, assessments, wound care, medication set up, catheter support and education.
  • In this role, you will get to work business hours (Monday-Friday, 8AM-4:30PM).
  • In this role, you will get to work less weekends!
  • This role does not require regular visits to the home office in Roseville, MN.
  • Driving is a must in this role. This role includes mileage reimbursement. Mileage and drive time are paid in between clients.

Responsibilities:
The Registered Nurse (RN) is responsible for the provision and monitoring of high quality clinical care and services to residents consistent with regulations and established clinical best practices.
Qualifications:
  • Registered Nurse with current licensure with the State Board of Nursing in state(s) in which he/she practices.
  • Current CPR certification.
  • Interest in gerontological nursing, rehabilitative and restorative clinical practices
  • Must have valid driver's license in state of residence and proof of auto insurance.
  • Demonstrated compatibility with PHS's mission and operating philosophies.
  • Demonstrated ability to read, write, speak and understand the English language to communicate with all customers.
