What is a Home Health Care Nurse?
A Home Health Nurse, or Home Health RN, travels to a patient’s home to administer services and help patients maintain their independence while they safely heal at home.
What does a Home Health Nurse do?
Home Health Nurses provide a variety of tasks during patient visits depending on the specific plan of care and type of patient. Some tasks include:
- Acute & chronic disease state management
- Medication management
- Condition assessment
- Tailored, goal-oriented treatment plans
- Post-surgical rehabilitation
- Patient and caregiver education
- Pain management
- Wound care
What are the qualifications for a Home Health Nurse?
- Graduate of an approved school of nursing
- Strong assessment and care planning skills
- Ability to delegate care appropriately and supervise Licensed Practical Nurses
Do Home Health Nurses work alone?
Home Health Nurses are autonomous, but they are not alone. All field staff work with supportive clinical supervisors, and the RN is a vital part of the collaborative clinical team.
