Licensed Vocational Nurse (Office Support)

Job responsibilities include:

• Answers and assists with incoming calls from the field and office.
• Completes Electronic Medical Records (EMR) in a timely and efficient manner in accordance to department policies and procedures.
• Reviews patient records, to include most recent clinical notes, medication lists and care plan prior to giving directions.
• Coordinates the visits of team members to ensure patient visits are made according to the plan of care.
• Responsible for ensuring all unplanned visits are covered by appropriate team members.
• Accurately maintains the electronic schedule for all clinical team members.
• Assists with phone triage and medication refills when needed.
• Works as an Admissions Coordinator during times of high need, or position coverage.
• Initiates communication with Case Managers, IDT members, doctors and those necessary to communicate with in order to meet the needs of the caller.
• Integrates documentation into the medical record in accordance with LBH policy
• Assists other nurses/case managers as requested by the Nursing Supervisor in order to provide safe and complete coverage of all patients.
• Outstanding orientation and ongoing staff training and education

Minimum qualification requirements:

• Current license to practice as a Licensed vocational nurse in the State of California
• Valid Driver's License and proof of Auto Insurance Coverage
• Ability to respond to needs of patients/families/caregivers in varied settings and to function effectively in a nursing facility environment
• Demonstrated skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles
• Comfortable with computers and mobile technology (cell phone, electronic medical record)
• Ability to handle multiple priorities, documentation requirements and deadlines
