Housing Specialist
Job description
To provide support as needed to case management services to individuals in OASAS Permanent Supported Housing (PSH) and Transitional Safety Units (TSU).
Hours: Monday - Friday 8am - 5pm, weekends as needed, open to flexibility
Essential Job Functions:
- Maintain standards that apply to Confidentiality and 42 CFR/HIPAA.
- Maintain client records per OASAS standards and agency policy and procedures.
- Manage assigned case load using approved tools to pre-screen, conduct intake assessment, evaluate, and recommend care plans for potential clients.
- Refer and connect individuals to needed agencies, programs and resources
- Assisting with and Directly provide: case management; rehabilitations; personal assistance with daily livings skills; residential stability in housing; financial management and assistance in gaining access to appropriate public benefits and services; peer support; 24 hour/7 day on call staffing; and, help with establishment of household including, if apartment mates are involved, facilitating cooperative apartment-mate agreements on bill payments, division of household responsibilities and other matters.
- Conduct home visits at a minimum of once a month, with increased visits and referrals based on client need
- Complete monthly data utilization report to OASAS
- Developing Service Plans with updates in a quarterly basis
- Calculate rent for clients on annual basis
- Screen potential admissions for PSH and TSU program
- Coordinate with housing partners/landlords
- Initial point of contact and liaison with community housing partners
- Utilize multidisciplinary team from various Credo programs to collaborate care for client needs
- Utilize Evolv (electronic health record) and Foothold to complete and maintain client records, complete reports and scanning of materials.
- Provide services in a culturally- and linguistically competent and sensitive manner.
- Participate in supervision at least bi-weekly.
- Attend and participate in relevant training while pursuing/maintaining CASAC.
- Attend staff meetings, in-service and conference/trainings as assigned.
- Convey information inter/intra agency.
- Offering presentations and outreach for recruitment
- Maintaining resident charts/records, and documentation as required
- When needed, completing case closing documentation and discharge planning
Minimum Job Qualifications:
Associates Degree/equivalent experience in the field of chemical dependency/
other human services field or CASAC preferred
And
Must have expert knowledge of community-based programs, with at least one-year experience working with people affected by substance use disorder and homelessness
And
Support a Person-Centered Approach service model
And
Must subscribe to a Professional Canon of Ethics.
Other Job Requirements:
Must have reliable transportation for occasional use when an agency vehicle is not available. Must maintain a valid Driver’s License, as specified in the personnel policies. Must maintain all laws and policies concerning confidentiality, security, and client rights as specified in the employee handbook, 42 CFR Part 2 and HIPAA. Must maintain current certification and/or training as specified by state law and agency regulation.
