Summary of the main function/purpose of the position:

Provides empowerment-based advocacy, safety planning, supportive counseling, crisis intervention and service management to Rapid Re-Housing participants suffering domestic violence and/or homeless survivors of domestic violence and referred by shelter, outreach services or other community partners. Provides information, referrals, and counseling to survivors. Coordinates services with the participant regarding their housing goals.

Principal duties/responsibilities:

Provides supportive counseling, service management, advocacy, and crisis intervention for all approved rapid re-housing participants. Meets with participants on a regular basis for service management.

Assists with the interview process and intake process for potential and new Rapid Re-Housing participants.

Establishes and maintains relationships with landlords, property manager, and agencies providing housing resources.

Works with participants to secure safe and stable housing.

Assists participants with working on educational and employment goals when necessary in support of housing stability.

Manages all aspects of assigned cases and acts as focal point with primary responsibility for services rendered to those participants.

Service management and documentation meets FCADV (Florida Coalition Against Domestic Violence), THHI (Tampa Hillsborough Homeless Initiative) and FESG (Florida Emergency Solutions Grant) program standards.

Provide on-going safety planning with participants.

Reviews service plan with participants on a regular basis.

Refers participants to community resources and other organizations based on the participant's needs.

Maintains participant files and ensures data is entered into the agency software system immediately after all participant contact. Maintains written records required for statistical reporting and programmatic objectives. Prepares and provides statistical information monthly.

Documentation for service notes will meet FCADV, THHI and FESG program standards.

Prepare records and adhere to funder monitoring requests and requirements.

Adheres to confidentiality requirements.

Attends ongoing case conference meetings.

Must be willing to work flexible hours, including nights and weekends, to meet participant needs.

Ensures reporting and documentation is captured and tracked for all participants.

Submits reporting documentation to meet funding requirements on a monthly and annual basis.

Develop and maintain community contacts to provide referral sources for participants.

Attend required agency and community meetings in support of the program, funders and collaborative partnerships.

Performs other related duties as assigned.

Bachelor's degree in social work or related field required plus 1 year of related experience.
