- Description: Discharge Lounge Specialist
Medical Assistant
The Discharge Lounge Specialist in Care Management, working as a member of the interdisciplinary team, provides assistance and support to all inpatients needing to transfer to the discharge lounge. The DCL specialist supports Care Management and Mission Control with throughput and capacity management. This position functions under the direct supervision of the Admin Supervisor and Care Integration Manager. Direct face to face interaction with patients and/or families in a hospital room is required.
JOB Responsibilities:
Discharge Lounge
a. Review the patient manager tile to determine capacity
b. Connect with the Clinical Expediter to screen patients
c. Screen patients to ensure they meet the inclusion criteria for the hospital discharge lounge.
d. Connect with the unit and nurses to ensure all discharge pieces are in place
e. Educate the patient and treatment team on the benefits of the discharge lounge
f. Escort the patient down to the lounge, providing one person assist as needed
g. Provide close supervision of the patient
h. Complete meds-to-beds (lounge) process as needed
i. Ensure patient has resources in the lounge – food, drinks, etc.
j. Update discharge lounge tracking sheet
k. Attend weekly workgroup on the discharge lounge
l. Present to various stakeholders on the discharge lounge
m. Create any power points, education or resources related to the lounge
n. Perform other tasks as assigned
Discharge Planning :
a. Deliver discharge information packets to nursing units for patients transferring to other facilities.
b. Schedule discharge transportation under direction of Case Manager or Care Integration Specialist (CIS)
c. Set up appointments for patients under the direction of Case Manager or Care Integration Specialist (CIS)
d. Deliver DME to patients under the direction of the Case Manager or Care Integration Specialist (CIS)
e. Create packets under the direction of Case Manager or CIS
f. Performs other tasks as assigned
REQUIRED:
MA certification received from a nationally recognized and accredited certifying body, upon hire or by the completion of the probationary period or internal job change evaluation period, as appropriate.
Currently, these include:
o The American Association of Medical Assistants (AAMA), awarding the Certified Medical Assistant (CMA.)
o The American Medical Technologists (AMT), awarding the Registered Medical Assistant (RMA.)
o The National Center for Competency Testing, awarding the National Certified MA (NCMA.)
o The National Health Career Association, awarding the Certified Clinical Medical Assistant (CCMA.)
o The American Registry of Medical Assistants, awarding the Registered Medical Assistant (R.M.A.) – for those qualifying through #2 above only.
Job Type: Contract
Pay: $25.00 - $30.00 per hour
