We are a home health agency established in 2004. Our Offices are located near the West Kendall & Tamiami Trail (SW 8th Street) area and we serve Miami-Dade county. We are seeking an experienced Intake Coordinator to join our team. The Intake Coordinator will be responsible for managing the intake process for new clients and ensuring that all necessary information is obtained to provide quality care.

Responsibilities:

• Answering incoming calls from potential clients, their families and other patient referral sources to gather information and answer questions about our services
• Conducting initial screenings of clients to determine their eligibility for home health services
• Coordinating with healthcare providers, case managers, and insurance companies to obtain authorization for services
• Scheduling and coordinating the initial assessments for new clients
• Ensuring that all necessary documentation is obtained and completed in a timely manner
• Serving as the primary point of contact for clients and their families throughout the intake process

Requirements:

• High school diploma or equivalent; associate or bachelor's degree preferred
• At least 1 year of experience in a healthcare setting, preferably in home health or hospice
• Knowledge of Medicare and Medicaid regulations and requirements
• Excellent communication and customer service skills
• Ability to work independently and as part of a team
• Strong attention to detail and ability to multitask
• Proficiency in Word, Excel and electronic medical record systems
