Position: Licensed Master Social Worker - LMSW
Department: Social Work - General medicine
Hourly Rate: $62

Locations: 5 shifts per week (days vary)

  • Bronx - 9am -5 pm
  • Manhattan 8am - 4pm, two days or two nights
  • Queens - 9am -5 pm
  • Brooklyn - 9am -5 pm

Requirements:

  • Active NYS LMSW
  • Master's Degree
  • Proof Of Full Covid-19 Vaccination + Booster if eligible
  • 1 year of LMSW Experience

Responsibilities:Physically meet the members where they are to gain a deep understanding of their situation and needs Problem solve member’s problems and needs: clinical, psychosocial, financial, environmental Engage members in a collaborative relationship, empowering them to manage their physical, psychosocial and environmental health

  • Prepare a member-oriented plan of care with members, caregivers, and health care providers
  • Insure member caregiver understanding as it relates to language barriers, stress reaction or cognitive limitations/barriers using verbal and nonverbal techniques
  • Advocate for members by assisting them to address challenges, and make informed choices regarding clinical status and treatment options
  • Maintain accurate, comprehensive and current clinical and non-clinical documents

Job Types: Full-time, Contract

Salary: $62.00 per hour

Schedule:

  • 8 hour shift
  • Day shift
  • Evening shift
  • Monday to Friday
  • Rotating shift

Experience:

  • Hospital social worker: 1 year (Required)

License/Certification:

  • LMSW LICENSE (Required)
