Aurora Mental Health Center (AuMHC) is seeking a Full-time Licensed Practical Nurse to work in our Assertive Community Treatment (ACT) program. About the team.
The Assertive Community Treatment team uses a person-centered approach with clients to build rapport and provide consistent support in the clients' lives. The ACT team services include case management, therapy, transportation, psychiatry, and nursing support.
Our Medical team consists of Psychiatrists, Psychiatric Mental Health Nurse Practitioners, RNs, LPNs, Medical Assistants, Licensed and Unlicensed Therapists, Case Managers, Milieu Counselors, and Peer Support Specialists.
$3,000 Sign-on bonus Available! *Not eligible for internal candidates.
Based on full time (40 hours/week). Prorated if fewer than 40 hours/week. Paid in two installments: 50% on first paycheck, 50% on paycheck following 6-month anniversary.
Salary for this role. Wage is based on licensure and company equity.
- $22.86 - $34.28 hourly
Bilingual language differential pay. *Rate depends on language requirements for the position and candidate's fluency.
- 10% of base salary up to a maximum of $6,000/year
About the role.
The ACT LPN is an active member of the care team and provides high-quality, skilled nursing services, including: patient education about mental illness diagnosis and symptoms, independent living skills, illness management education, crisis stabilization and prevention, information and/or referral for community resources. Nursing responsibilities include: performing nursing assessments, passing medications, and collaborating with members of our multidisciplinary team while following clients' treatment plans.
Responsibilities:
- Support the clinical functions of the medical team, completing all medical orders and communicating vital patient care information to physicians/prescribers.
- Perform nursing interventions for which an LPN has been trained.
- Observe, identify and monitor condition of patient, including changes in symptoms, and relate same to appropriate members of the treatment team.
- Administer injections and medications, as required.
- Provide medical case management and other clinical interventions, as appropriate.
- Act as a liaison to outside agencies and providers to ensure continuity of care for patients.
- Provide assistance in crisis intervention, as needed.
- Accurately and completely chart interventions, lab results, medications dispensed, and all activities related to patient interactions according to the organization's procedures.
- Provide information to credentialing, payroll, etc. in a timely manner.
- Monitor and coordinate care for assigned clients in partnership with the other members of the client's care team.
Requirements:
- Licensure: Active State of Colorado Registered or Practical Nursing License.
- Associate's degree.
- Preferred: Bachelor's degree.**
- Four years of relevant clinical experience.
Required Vaccinations and TB Test.
AuMHC has prioritized the health and safety of our clients and staff and requires all employees to be fully vaccinated against Covid-19, receive an annual influenza (flu) shot, and receive negative TB test results prior to their first day of hire. Proof of both vaccinations and negative TB test will be required upon your first day of employment. Medical and religious exemptions may be requested for either COVID 19 vaccine or annual flu shot.
