Position Description:

The nurse's responsibility includes collecting client health data and working with the patient/family in the patient's home. Must work closely with RN's and teams to achieve goals. Qualified candidates must be committed to providing outstanding care and possess the following qualities: detailed oriented, honesty, flexible, organized, open-minded, be a team player, and have proficient computer skills. This position is a permanent full-time position.

Educational Requirements:

• One-year experience as an LPN required
• Homecare experience preferred

Requirements:

• One weekend every 3-4 weeks
• Strong communication skills
• Exceptional assessment skills
• Proficiency in medical EMR preferred
• Driver's license in good standing, reliable transportation and vehicle insurance are required
• Ability to provide trusted, compassionate and expert care
