Forest View Hospital, located just seven miles southeast of downtown Grand Rapids, Michigan, is a private 108 bed psychiatric facility that serves children, adolescents and adults. We are licensed by the State of Michigan, fully accredited by The Joint Commission and we bring more than 45 years of experience to the evaluation, diagnosis and treatment of a wide range of behavioral health problems. As a part of our continued success and growth, we are seeking qualified applicants for a Full Time Inpatient Clinician.

SOCIAL WORKER (LICENSED THERAPIST) - $5,000 sign-on bonus

NOTE: This is not a remote position.

POSITION DESCRIPTION:

The Inpatient Clinician provides psychiatric case-management and therapeutic services to assigned patients with diverse needs to ensure there is a coordination of care between the hospital, community, treatment, therapeutic services, education and discharge planning.

Key Job Duties and Responsibilities:

• Completes a thorough psychosocial assessment and collaborates with the patient to develop individualized treatment goals.
• Initiate discharge planning and arrange aftercare appointments/referrals.
• Oversee the treatment planning process with person-centered planning and will coordinate all disciplines in treatment interventions, updating treatment plans every seven (7) days or as needed.
• Conducts group therapy and other groups as assigned.
• Facilitate individual and/or family sessions as needed.
• Documents each interaction with patient that is consistent with hospital standards.
• Attends all appropriate team meetings as required.

Requirements:

• Must have one of the following licensures: LMSW, LLMSW, LPC, LLP, TLLP
• State of Michigan Licensure.
