The Lifestyle Coach/Registered Dietitian is an integral part of the NARA WISEWOMAN cardiovascular disease prevention (CVD) program. Staff working under the WISEWOMAN grant work closely with NARA’s other chronic disease prevention programs including the Women’s Wellness Program and the Diabetes Treatment and Prevention Program. Individuals in this position are responsible for guiding program participants through a goal making process while involving lifestyle modifications that will lower cardiovascular disease risks.

This position will assist with the coordination of activities including CVD risk assessment, teaching, scheduling, and reporting outcomes of the WISEWOMAN Healthy Behavior Support Services and related clinical measures. The Lifestyle Coach will take part in creating, conducting, and implementing health promotion activities such as Diabetes Prevention Program classes, walking programs, cooking presentations, and employee health activities. An ideal candidate will be someone who role-models wellness.

The Lifestyle Coach will be responsible for a panel of patients and, in collaboration with primary care providers, help patients meet their preventive, chronic and acute care needs related to CVD prevention. Part of engaging patients and encouraging them to take an active role in their health will involve providing necessary tools to make healthy lifestyle choices and adopt lifelong healthy behaviors.

Primary Responsibilities:

CLINICAL:

  • Take and evaluate patient vitals including blood pressure, capillary blood glucose, weight, height, and point of care lab measures including A1C and Cholesterol.
  • Provide health education, resources and tools to help patients establish sustainable lifestyle risk reduction.
  • Capture relevant information about the patient's health and perform standard age and condition-appropriate screening assessments.
  • Develop culturally sensitive recommendations which take into consideration socioeconomic status and personal preferences.
  • Communicate regularly with primary care providers and other chronic disease prevention programs at NARA.

COACHING:

  • Establish supportive relationships with patients and their families.
  • Conduct health coaching using evidence-based communication strategies (ex: Motivational Interviewing, Cognitive Behavioral Therapy, etc.).
  • Participate in trainings to achieve proficiency in health coaching skills necessary to lead patients towards healthy behavior change.
  • Communicate with patients by phone, email, and text to provide outside support and encouragement.
  • Provide guidance to patients when goal setting and creating sustainable wellness plans.
  • Coordinate with other team members to navigate patients to follow-up care when referred services are necessary.
  • Collaborate with other team members to facilitate group events and classes for patients.

OPERATIONAL:

  • Assist with the coordination of care by taking part in scheduling of appointments, coordinating referrals, and sharing or transferring information with the patient's care team.
  • Collect and report data required for WISEWOMAN grant continuation.
  • Contribute to open team forums on problems and process improvements by listening, sharing input and ideas, and enacting decisions and plans.

MINIMUM JOB QUALIFICATIONS:

  • Minimum of a Bachelor’s degree in a health related field (Human Nutrition, Exercise Physiology, Nursing, and Public Health).
  • Registered Dietitian with license to work in Oregon.
  • Experience in coordinating health behavior change, health coaching, and health education.
  • Proficient computer skills including Microsoft Office Excel, Word, and PowerPoint.
  • Ability to work a flexible schedule, including Saturday programs, weekday early mornings and at times, late evenings.
  • Must have access to a reliable vehicle and a valid Oregon drivers’ license and be willing to travel between 2-3 different sites within the Portland Metro area.
  • Basic knowledge of disease prevention (cardiovascular and diabetes) and understanding of exercise principles, nutrition and weight management.

PREFERRED QUALIFICATIONS:

  • Experience working with American Indian/Alaska Native populations.
  • Experience with the National Diabetes Prevention Program.
  • Certification by a nationally recognized coaching program.

Job Type: Full-time

Pay: $65,000.00 - $75,000.00 per year
