Position Title: LPN Care Coordinator
Job Overview: The LPN Care Coordinator promotes excellence in healthcare delivery to the patient and/or patient family. Under supervision of the RN Care Manager, the LPN Care Coordinator helps patients by following up on their comprehensive care plans. The LPN Care Coordinator has a strong role to play in disease management, care coordination, and utilization management for patients within the Primary Care teams. The LPN Care Coordinator will work to support patients with navigating the healthcare field, connect people to community resources, coordinate referrals, and assist with prior authorizations, patient assistance programs, and obtaining DME.
This position is responsible for helping to manage high-risk patients with multiple comorbidities or at high risk for hospital admission or readmission. The RN Care Manager will develop care plans in conjunction with the Care Teams, and the LPN Care Coordinator will assist with implementing interventions from the care plan that help move the patient toward optimal health while maintaining clinical best practices.
Shift: Full Time, Monday through Friday 8am to 5pm. Shifts may change with patient needs or program expansion.
The LPN Care Coordinator may perform the following duties:
  • Partners with all clinical teams as a resource on care management.
  • Identifies targeted patient populations that would benefit from care management.
  • Pre-visit planning workflow to ensure care completion prior to visit whenever possible.
  • Coordinates patient care services for selected patients across the continuum of care through collaboration with the patient and family and health care providers in achieving optimal patient outcomes.
  • Communicates with patients after hospitalizations to schedule appointments, assist with medication education, review after-visit summary and provide the patient with a point of contact.
  • Collaborate with the Care Team and follow up appropriately to ensure smooth transitions of care for patients treated in a facility (inpatient or emergency department), by a specialty physician, or by another health care provider.
  • Involve patients in activities to improve their health (patient engagement) and conduct health teaching, educate on self-management tasks, following care plan set in place by RN Care Manager.
  • Identify barriers preventing patients from achieving optimal health, problem solve and plan with the Care Team and outside resources to overcome.
  • Lead health Care Team by influence and role modeling integrated effective nursing practices, excellent customer service, innovation and providing outstanding support for the physician practice.
  • Ensure safe practices and report any concerns
  • Ensure complete and accurate documentation in the medical record
  • Collect and monitor data related to outcomes.
  • All other duties as assigned.

Required Skills and Knowledge:
  • Able to effectively communicate with others, both written and orally.

  • Ability to prioritize work, manage multiple demands, and demonstrate successful time management skills.
  • Excellent computer skills.
  • Ability to work both with and for the team.

  • Must be covid vaccinated or willing to do so.
Education: Completion of a licensed practical nursing degree program. Must hold an active and unencumbered Licensed Practical Nurse license in the state of Oregon.
Experience: Minimum of 3 years experience as a Licensed Practical Nurse, preferably in a care management setting. Recent clinical experience is desired.
Certification: Current healthcare level CPR certification required. Attend continuing education which may include some out of town sessions. Current Oregon Driver License in good standing along with reliable and insured transportation.
