JOB DESCRIPTION: LICENSED PRACTICAL NURSE Reports to: Case Manager and Home Care Coordinator
Job Summary:
As a member of the interdisciplinary Team, the LPN works under the general direction of the Case Manager (RN) on duty and the attending physician in the provision of skilled nursing assessments, planning, and care in order to maximize the comfort and health of patients and families The LPN is responsible for identifying patient/family needs and for providing supportive care in accordance with the attending physician's orders, plan of care, and policies and procedures of Heritage Hospice under the direction of the Primary Nurse Qualifications:
Graduation from an accredited school of nursing with a current registration in the State of Georgia.
Minimum of two years’ experience in medical-surgical, oncology or community health nursing or equivalent experience
Demonstrated ability to assist with and respond to the needs of patients and families in varied settings.
Skilled in nursing practice, able to cope with family emotional stress and tolerant of individual lifestyles.
Prior hospice experience preferred
Responsibilities:
Assist the Case Manager with responsibility for a patient/family caseload that includes the assessing, planning, implementing, and evaluating phases of the nursing process.
Initiate communication with attending physician, other hospice staff members, and other agencies as needed to coordinate optimal care and use of resources for the patient/family.
Maintain regular communication with the Case Manager (RN) to review caseload.
Maintain regular communication with the attending physician concerning patient/family caseload.
Stay abreast of the physical, psychological, social, and spiritual factors that may influence patient/family health status and communication changes so that they may be incorporated into the plan of care
Maintain up-to-date patient records so that problems, plans, actions, and goals are accurately and clearly stated and that changes are reflected as they occur, Seek input from other team members regarding the patient plan of care to obtain additional knowledge and support.
Accept responsibility for coordinating physical care of the patient by teaching other caregivers.
Meet regularly with hospice team to review problems or unique issues from caseloads, share professional support, and exchange feedback aimed toward enhancing professional growth
Inform the Case Manager (RN) of possibility for potentially problematic patient/family issues.
Provide appropriate support at time of death and period of bereavement. Participate in orientation and in-service training programs for professional staff as appropriate.
Demonstrate familiarity with policies of Heritage Hospice and the rules and regulations of state and federal bodies which aid in determining policies.
Maintain consistent and regular attendance with few, if any, absences. Accept other assignments as appropriate
