Job Description: LPN (Licensed Practical Nurse)

  • Full Time 6P-6A w/ rotating weekends

The licensed practice nurse (LPN) is responsible for rendering nursing care in assigned area in terms of individualized client needs, according to dependent and independent nursing functions and conformance with recognized nursing techniques, procedures, and established standards based on the scope of practical nursing, under the direction/delegation of the registered nurse

  • Utilize nursing knowledge and skills in the safe implementation of basic preventative therapeutic and rehabilitative nursing care of assigned patient as evidenced by documentation and observation of positive patient care outcomes
  • Collect and document the patient's current health status and medical history. Complete and/or review Admission Data Record as defined by policy. Make accurate and ongoing assessment of patient's status and respond appropriately
  • Perform delegated independent nursing functions using established procedures, policies, guidelines and standards as observed by the registered nurse
  • Administer medication accurately, observing patient response, as evidenced by documentation in the medical record
  • Maintain and coordinate communication among patient, staff and physician
  • Accurately transcribe physician orders according to policy and procedure
  • Perform direct patient care, utilizing established procedures, policies, and standards
  • Work collaboratively with other members of the health care team in coordination of the patient's care
  • Assist other Team Members in the performance of their assignments
  • Other duties within the scope of practice as assigned

Minimum Qualifications:

  • Valid, unrestricted LPN license
  • Current CPR
  • Ability to pass a background check
