Function: Under limited supervision, provide and maintain a high level of care for the individuals that reside at the ICF/ID and CTH II homes.

Job Duties

1. Completes healthcare documentation to federal, state, and local guidelines and doctor’s orders.

A. Completes all assessments for single plans and prepares reports monthly to summarize the health care status of all people in residence.

B. Documents nursing activity in the nurse’s notes and medication administration on the Medication Administration Record.

C. Charts weekly/monthly weights and vital signs for consumers.

D. Obtains physician orders as needed, within 24 hours notes orders received and obtains signature from physician for verbal orders within 48 hours and conducts Monthly reviews of orders to assure accuracy.

2. Provides medical care according to federal, state, local guidelines, and doctor’s orders.

A. Administers medications and treatments as prescribed by the attending physician.

B. Completes controlled drug counts daily for each administration of the medication and reports discrepancies to supervisor and to agency administration.

C. Respond in a timely manner to calls from employees providing care to those served and then conducts assessments, over the phone and in person, of illnesses and injuries, decides treatment or physician services needed and arranges services to meet recommended actions as a result of assessment.

3. Performs routine nursing responsibility.

A. Accompany consumers to doctor’s appointments, laboratory appointment and other appointments as necessary.

B. Check all medications in nursing cabinets for re-ordering of medications or discarding all expired medications and takes action to assure medications are in stock and available at the residence as ordered.

D. Keeps track of, arranges, completes actions to schedule routine and emergency health care screenings, consultations and as needed physician or therapist office visits and in or out patient hospital services for those being served.

4. Oversee direct employee health needs.

A. Administers TB skin tests for consumers and Hepatitis B and TB injections for staff.

B. Maintain employee health records to assure pre-employment physicals, drug screens and TB tests are completed as required.

5. Participates in mandatory meetings for those served, including Support Plan meetings, Psychiatric Clinics, Human Rights Committee, staff meetings, Safety Committee, or other planning meetings are assigned.

A. Thoroughly reviews past year’s health status and needs, action taken and progress made toward meeting those needs and prepares a written report for the Case Manager/QIDP two weeks in advance of the plan meeting and files reports in charts.

B. Attends planning meetings in person, verbally reports assessment and recommendations, participates in discussions of person’s needs and recommendations to meet needs, and collaborates with others present to finalize the Plan for those being served.

6. Assist with training upon request.

A. Designs and conducts medication administration training for new employees and for those needing refresher training.

B. Designs and conducts training for employees related to health issues such as signs and symptoms of illness, CPR/First Aid, blood Bourne pathogens, and medication side effects.

7. Responds to on-call.

Serves as on call for emergencies rotating with other nurse(s), responding to requests for assistance within 15 minutes of call.

8. Other related duties as assigned.

A. Performs other related duties in a timely and accurate manner.

Minimum Requirements: Current LPN license. Must be in good standing with the SC Board of Nursing. Valid SC driver’s license.

Physical Requirements: Reading, writing, driving, standing, walking, sitting, lifting, carrying, pushing, pulling, climbing, balancing, stooping, kneeling, crouching, crawling, reaching, handling, speaking, hearing, seeing, depth perception, and color vision.

Skills and Ability Requirements: Knowledge of practical nursing procedures. Knowledge of sanitation, personal hygiene, basic health and safety practices. Candidate must have knowledge of the human anatomy, ability to make accurate technical and physical observations of patients and ability to accurately record observations.
