Overview:
The Brookdale FOCUS Project will incorporate HIV and HCV testing into routine care across Brookdale’s nine clinical sites (including seven ambulatory care settings, the Emergency Department, and the inpatient service), with the goal of increasing the total number of HIV and HCV tests, thereby identifying previously undiagnosed individuals earlier in the stage of disease. The FOCUS Project Coordinator will oversee management of the project, supervision of staff, monitor achievement of objectives and ensure compliance with funder requirements.
Responsibilities:
  • Oversee all monitoring, reporting and quality management activities of the Brookdale FOCUS Project
  • Orient, train and directly supervise Patient Navigators and Data Manager on the FOCUS Project.
  • Conduct ongoing review of Patient Navigators, including face-to-face evaluation of staff competency; chart -based review of staff work to ensure quality of service delivery; and review of field-based encounters.
  • Hold monthly staff meetings to address any trends or systemic problems in patient workflow or staff training needs.
  • Facilitate interdisciplinary case conferences with STAR Brookdale care team.
  • Provide backup to Patient Navigator on-site and in the field, as needed.
  • Participate in multidisciplinary case conference meetings and other STAR Brookdale meetings as required.
  • Develop a system to monitor staff productivity; review achievement of program objectives monthly and develop a system to revise procedures when objectives are not met
  • Serve as the primary liaison with multiple departments as it relates to identification of patients with HIV and HCV, linkage to care and increasing patient accessibility to care and services.
  • Maintain regular communication with Data Manager regarding data submission and trouble shoot any problems to improve the data submission to funder.
  • Ensure completion of data reports by the Data Manager, complete and submit the monthly program reports to funder.
  • Ensure coordination of resources and logistics for ongoing staff trainings in collaboration with the STAR Health Service Chief of Service and Manager.
  • Maintain ongoing communication with STAR Brookdale leadership to ensure coordination and integration of new services within STAR Brookdale and the larger Brookdale community.
  • Act as a liaison between the Brookdale FOCUS project and the funder project officer.
  • Ensure that staff adhere to Safety in the Field guidelines
  • Participate in STAR Program outreach and education efforts regarding HIV, HCV, behavioral health issues and promotion of services.
  • Adhere to Brookdale’s Code of Ethics and Conduct
  • All other duties as assigned.
Additional Requirements
  • Position will require travel to SUNY Downstate Medical Center for staff meetings and mandatory training.
  • Position must adhere to SUNY Downstate Medical Center’s policies and procedures related to confidentiality of HIV related information; mobile device policy; and obtaining consent for texting and emailing clients.
Qualifications:
Education:
  • Bachelor’s degree and at least 5 years' case management or related experience and 2 years’ supervisory experience.
  • Master’s degree in public health, social work, nursing or related field strongly preferred and may substitute for 1 year of experience.
Experience:
  • Experience working in multicultural, team-oriented setting preferred
  • Experience managing grant funded projects strongly preferred
Knowledge and Skills:
  • Effective written, verbal, organizational and interpersonal skills.
  • Strong analytical skills, experience using electronic health record and database systems strongly preferred.
Physical Requirements:
  • Exerting up to 25 pounds of force occasionally and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body.
  • Sedentary work involves sitting most of the time. Jobs are sedentary if walking and standing are required.
