Position Overview

The Ministry Associate will further the purpose of Segue Health to assist in transitioning patients to their home by providing comprehensive care focused on good physical, mental, and spiritual health by focusing on addressing their mental and spiritual concerns. This will be accomplished by regular, direct contact with patients, their families, their caregivers, and Segue health providers.

Principal Duties and Responsibilities

· Receive and respond to ministry requests from Nurse Practitioners

· Research and order items requested by Nurse Practitioners

· Contact vendors (pharmacies, medical supply companies, etc.) to arrange payment for necessary items

· Communicate outcomes with Nurse Practitioner and Segue management

· Document and track ministry activity in STARS

· Maintain database of vendors, community agencies, churches, and other ministries capable of assisting patients

· Contact patients via phone, text, letter, or other means for ongoing ministry

Qualifications

· Active church involvement and ministry participation

· A heart for serving others

· Comfortable interacting with medical professionals and patients

· Good communication skills

· Attention to detail and good organizational skills

· Knowledge of Microsoft Office and ability to learn EMR/EHR software

· Ability to work remotely, but must live in Memphis, TN or north MS

Job Type: Full-time

Pay: $30,000.00 - $35,000.00 per year
